A Case Report of Metastatic Gallbladder Cancer as The Cause of Acute Appendicitis

Abstract: Appendiceal metastasis from primary gallbladder cancer is rare. Here we report the case of a 62-year-old female with stage IV gallbladder cancer who presented to the ED four months after initiation of her cancer treatment. Diffuse abdominal pain was her only presenting symptom and contrast-enhanced CT imaging revealed findings consistent with acute appendicitis. Prompt surgical treatment with laparoscopic appendectomy was initiated. Surprisingly, the histologic analysis was consistent with gallbladder primary … Show more

“…Primary appendiceal neoplasms are incidentally found in 1-2% of appendectomies, with mucinous neoplasm (0.6%) and carcinoids (0.3-0.9) being the most common lesions [ 10 , 11 ]. Metastatic involvement of the appendix presenting as acute appendicitis is an even rarer entity, with few case reports documenting such incidences from carcinoma of breast, ovary, prostate, lung, kidney, stomach, colon, and hepatobiliary tract [ 12 18 ]. Unlike the growth pattern typical of primary appendiceal carcinoma, which starts from the mucosa and progresses outward, metastasis to the appendix involves the serosa first, followed by inward infiltration [ 14 , 16 ].…”

“…There are only two case reports found in the literature documenting gallbladder cancer metastasis to the appendix and only one of them presented as acute appendicitis clinically [ 16 , 18 ]. Another case report described a metastatic cholangiocarcinoma mimicking acute appendicitis [ 17 ].…”

“…There was one low-grade serous ovarian carcinoma metastasis [67] causing perforated appendicitis. The other cases included hepatocellular carcinoma [68], choriocarcinoma of the mediastinum [69] granular cell tumor [70] acute promyelocytic leukemia [71], nasopharyngeal carcinoma [72], non-seminomatous testicular cancer [73], testicular seminoma [74], inflammatory myofibroblastic tumour [75], two cases of Kaposi's sarcoma [76,77] a natural killer cell/ T cell lymphoma [78], two cases of appendiceal schwannoma [79,80], two cases of cholangiocarcinoma [81,82], a case of metastatic gallbladder carcinoma [83,84], and two cases of pancreatic adenocarcinoma [85,86]. In our results, there was a total of 46%(n=34) of individuals with complicated appendicitis, 54%(n=40) with uncomplicated appendicitis, and 4 cases did not have degree of inflammation documented.…”

“…En una revisión de la literatura, que evaluó retrospectivamente los hallazgos histopatológicos de apendicetomías, solo 7 pacientes evidenciaron compromiso metastásico, de un total de 80 698 (7) , cifra muy significativa y que pone en perspectiva lo inusual de dicha presentación. De los sitios primarios de metástasis en este estudio no se reportó la vía biliar, siendo esta una presentación atípica (8) .…”
