A clinical comparison of unrelated cord blood transplantation and unrelated bone marrow transplantation for adult patients with acute leukaemia in complete remission

Abstract: Summary. We performed a clinical comparison of unrelated cord blood transplantation (UCBT) and unrelated bone marrow transplantation in adult acute leukaemia patients in complete remission (CR) who received the same conditioning regimen, graft-versus-host disease (GvHD) prophylaxis and supportive treatment. The incidence of acute GvHD was almost the same between the two groups, but the haematopoietic recovery was delayed and the incidence of chronic GvHD was higher in the UCBT group. The probability of 2 year … Show more

“…This may be one of the possible reasons for our favorable result. Although several studies have suggested the promising results of unrelated CBT for adult patients, [16][17][18] the role of unrelated cord blood as an alternative stem cell source is not well defined in adult MDS patients. Therefore, we updated the results of unrelated CBT for adult MDS patients.…”

“…More rapid hematopoietic recovery in UCB recipients of higher cell doses was reported. [14][15][16] Wagner Table 3 Univariate analyses of transplant-related parameters in the cohort of 65 patients Patient characteristics n Graft failure Acute GvHD Xgrade II a Transplant-related mortality at day 100 b 6-year survival b n/n (%) P n/n (%) P n /n (%) P n /n (%) P et al 13 noted a high survival rate in recipients of no more than two HLA-mismatched UCB grafts that contained at least 1.7 Â 10 5 CD34 þ cells/kg of recipient body weight. Gluckman et al 17 observed no correlations of the number of HLA disparities to the treatment-related mortality and the overall survival of UCB recipients, despite the fact that the higher the number of HLA disparities, the higher the probability of acute GvHD grades III and IV and the lower the risk of relapse.…”
