A clinicotherapeutic analysis of Stevens–Johnson syndrome and toxic epidermal necrolysis with an emphasis on the predictive value and accuracy of SCORe of Toxic Epidermal Necrolysis

Abstract: Serial analysis using SCORTEN is better than analysis performed only on day 1. Although the scale is largely useful, its performance is influenced by the demographic profile of the study population; minor modifications based on the population studied may increase the predictive accuracy of the original SCORTEN. In addition, conservative management is a valid therapeutic option, is preferable to treatment with steroids and immunosuppressants, and is highly recommended.

“…Several authors studied the performance of the SCORTEN score over time, performance seems to be related to time since onset of the disease. Bansal hypothesized that SCORTEN will perform best at its peak . Because of the differences in admission delay, the day of best performance varies in the literature between day 1 and 5 of admission with AUC's above 80%.…”

“…Severity of Illness Score for Toxic Epidermal Necrolysis (SCORTEN) score has been developed to evaluate the severity of TEN and predict its mortality[5]. Since this system is proven to be reliable and applicable for estimating risk for death among both SJS and TEN patients[68], doctors routinely calculate the score after the admission. Meanwhile, numerous studies set SCORTEN-derived expected mortality as internal control to evaluate the effects of immunomodulatory therapy on death prevention[912].…”
