A late complication of transobturator tape procedure: vaginocutaneous fistula formation with vaginal mesh erosion

Abstract: Transobturator tape procedures are a common treatment modality in patients with stress urinary incontinence (SUI). Various complications have been reported related to this procedure. We present a rare case of vaginocutaneous fistula formation 5 years after transobturator tape surgery. A 56-year-old woman presented with complaints of increased vaginal discharge, difficulty in coitus, and serosanguineous discharge from the left groin 5 years after transobturator tape surgery for SUI. Pelvic examination revealed … Show more

“…The treatment of urinary incontinence in the last decade has been widely performed through transobturatory sling surgery, with high rates of positive outcomes. Although this procedure uses minimally invasive surgery principles, postoperative complications have been reported, including bladder dysfunction, urinary retention, erosion and extrusion of the synthetic mesh, and more rarely, abscess formation and fistulization 4 . No statistics were found concerning mesh infection in this type of procedure; for comparison, it is known that after laparoscopic surgical repair of inguinal hernia, the rate of mesh infection is lower than 0.5% 5 .…”

“…Fistulas are typically late complications, with rare reports of vaginocutaneous fistulas 4,6,7 and a single report of vaginofemoral fistula in literature 8 . We did not find any report of vaginocutaneous fistulas from abscesses caused by NTM, as reported in this study.…”

“…46,48,66,98,173,178,212,217,334 These fistulas are most frequently associated with bladder or urethral erosion of the sling and patients can present in a variety of ways, and as late as 6 years after the initial surgery. 41,43,44,[335][336][337][338] Despite their low reported incidence, the possibility of fistulas should be considered when patients present with recurrent incontinence, OAB, pain and/or voiding dysfunction after mesh surgery. 41 Not infrequently, a diagnosis of fistula can be obscured, owing to the presence of adjacent granulation tissue (Figure 4).…”

“…Many cases of mesh exposure and fistula formation have been reported in the litterature. [3][4][5] In this article we present a case of vaginocutaneous fistula and buttock abscess formation seven years after transobturator polypropylene tape insertion, an unusual complication with this type of tapes.…”
