ResumoA degeneração discal é uma condição que compromete as funções do disco intervertebral, podendo levar a vários processos patológicos importantes, como hérnias discais e estenoses de canal. Apesar de sua etiologia ainda ser desconhecida, cada vez mais estudos têm demonstrado o papel preponderante de fatores genéticos em detrimento de fatores ambientais. Com o objetivo de revisar o conhecimento atual sobre os genes associados à degeneração do disco intervertebral, foi realizada uma revisão narrativa da literatura inglesa nos últimos 10 anos sobre o tema. Concluímos que há uma série de genes que foram associados à degeneração discal em seres humanos, incluindo genes codificando colágeno I α-1 (COL1A1), colágeno IX (COL9A2 e COL9A3), colágeno XI (COL11A2), interleucina 6 (IL-6), agrecano (AGC1), receptor de vitamina D (VDR), metaloproteinase de matriz 3 (MMP-3), além de microRNAs. Dessa forma, a presente revisão enfatiza os últimos avanços na associação de genes com fenótipos de discos degenerados específicos, polimorfismos de nucleotídeos únicos, hereditariedade e interações genético-ambientais em relação à degeneração discal, com o intuito de permitir ao clínico entender esse mecanismo de degeneração e estar preparado para as novas terapêuticas que estão por vir baseadas na genética.
SUMMARY This study aimed to evaluate the efficacy of the action of the Lactobacillus Plantarum probiotic as a immunomodulatory and hypolipidemic agent in dyslipidemic nephrotic children and adolescents. METHODS: This is a randomized, double-blind, placebo-controlled clinical trial in pediatric, compensated or partially compensated nephrotic syndrome and dyslipidemic subjects undergoing regular outpatient follow-up. Serum lipid and TNF-α (proinflammatory) and IL-10 (anti-inflammatory) cytokine variations were evaluated. Cytokines were analyzed by enzyme-linked immunosorbent assay (ELISA). RESULTS: In the probiotic group there was a tendency to reduce TNF-α levels and increase IL-10 levels when compared to controls. Regarding the lipid profile, there was a decrease in serum triglyceride (6.0 mg / dL) and total cholesterol (41.5 mg / dL) levels in the probiotic group when compared to baseline levels, while in the control group there was an increase in serum triglyceride (49.5 mg / dL) and total cholesterol (8.0 mg / dL) levels, respectively. CONCLUSION: Preliminary results suggest that L. Plantarum showed an immunomodulatory and hypolipidemic effect in nephrotic and dyslipidemic pediatric subjects.

The average age of women with a breast cancer diagnosis was 55 years, and pertaining relationship status, a little over 42% fitted the not single category, and over 43% had no data. About the skin color, 37.06% were white and over 39% had not filled this variable on the record. About the tumor aspects related to the disease's extension, we identified that in over 48% of the cases the tumors were classified as localized, over 23% as regional and only 4.23% as distant. About the tumor's side, over 33% had no information, 31.04% were located at the right side and 33.40% at the left side. About the tumor's type most cases, 85.5%, were invasive ductal carcinoma, 4.6% ductal carcinoma in situ, 2.6% invasive lobular carcinoma and only 0.30% lobular carcinoma in situ. 106 cases had no information about the tumor's type. Conclusion: This study's goal was a better comprehension about this disease's effect on the patients: it's evolution, association with risk factors, presence of metastasis and mortality.
Objetivos: Estimar a taxa de transmissão vertical do HIV e os fatores de risco materno-fetais em crianças nascidas em 2015 em seguimento durante os anos de 2015 a 2017 no maior centro de referência para tratamento para HIV do estado de Goiás. Métodos: Estudo de coorte retrospectivo de 111 crianças expostas ao HIV nascidas em 2015 de mães HIV positivas. Resultados: Entre as mães, 85 (92,4%) utilizaram TARV durante a gestação. Das 92 crianças que mantiveram seguimento, 4 (4,34%) adquiriram infecção perinatal pelo HIV. 81 (88%) recém-nascidos fizeram uso de profilaxia antirretroviral. Um fator protetor importante de transmissão vertical do HIV foi a profilaxia antirretroviral do RN (OR = 0,02; IC 95%: 0,00-0,56; p = 0,04). Outros fatores investigados, como uso de TARV durante a gestação, não foram estatisticamente significativas para risco. Conclusão: A taxa de transmissão vertical do HIV ainda se encontra elevada no estado de Goiás e os desafios para sua prevenção consistem na perda de seguimento e falhas nas medidas estratégicas.
Yokenella regensburgei belongs to the family Enterobacteriaceae and is an opportunistic agent rarely associated with infections in humans. We report a case of osteoarticular knee infection caused by Y. regensburgei in a patient under treatment for rheumatoid arthritis, using corticosteroids, with complication in primary total arthroplasty of the knee. Y. regensburgei was identified using the VITEK2 system. Antimicrobial susceptibility testing was performed using the disk-diffusion method, according to the guidelines from the Clinical and Laboratory Standards Institute. The patient presented favorable clinical evolution after the second debridement, with complete removal of the prosthesis and antibiotic therapy with sulfamethoxazole/trimethoprim. This is the first case of Y. regensburgei infection described d in Brazil. KEY WORDS: Yokenella regensburgei; osteoarticular; infection; sulfamethoxazole; trimethoprim; prosthesis.
