A rare case report of very low thyroglobulin and a negative whole-body scan in a patient with a solid variant of papillary thyroid carcinoma with distant metastases

Abstract: Rationale:The early detection of recurrent differentiated thyroid carcinoma (DTC) cells in postsurgery DTC patients relies on the sensitivity of measuring both the level of thyroglobulin (Tg) and 131-iodine distribution on a whole-body scan (WBS). Recent studies have defined patients who subsequently have no evidence of disease as those who have a stimulated Tg level <1 ng/mL with no other radiological or clinical evidence of disease.Patient Concerns:A woman patient with solid variant papillary thyroid carcino… Show more

“…We hypothesize that these findings were not related to reduced Tg synthesis, since no evidence of poor differentiation of the primary thyroid tumour or dedifferentiation of lymph node recurrences was reported by the pathologist. The production by the tumour of a Tg variant which was not recognized by the immunoassay cannot be excluded but seems unlikely [ 15 ].…”

“…The number of reports on prognosis of patients with SVPTC is small, thus leaving the clinical implication of this histological subtype undetermined. Previous studies exhibited patients with SVPTC had a slightly higher incidence of metastases compared to patients with CPTC [5,6,60]. Another study showed that patients with SVPTC were at a significantly higher risk of relapse compared to other subtypes [61].…”

“…In contrast, it has been reported that a patient with an extremely low level of Tg after RAI ablation developed distant metastases in the brain and lungs, even when his WBS was negative. This report implied that it seemed unsafe to view Tg level or WBS as a single marker ( 91 ). Notably, the level of Tg after the first RAI ablation was also regarded as an effective decisive and prognostic indicator for the second RAI ablation ( 92 ).…”
