Acceptability of the nestorone®/ethinyl estradiol contraceptive vaginal ring: development of a model; implications for introduction

Abstract: Objectives Develop and test a theoretical acceptability model for the Nestorone®/ethinyl estradiol (NES/EE) contraceptive vaginal ring (CVR); explore whether domains of use within the model predict satisfaction, method adherence and CVR continuation. Study Design Four domains of use were considered relative to outcome markers of acceptability, i.e. method satisfaction, adherence, and continuation. A questionnaire to evaluate subjects’ experiences relative to the domains, their satisfaction (Likert scale), an… Show more

“…Research suggests that users are generally satisfied with the contraceptive ring, but rates of uptake remain relatively low. Women are more likely to be satisfied with the ring if they find it easy to use, they experience no side effects, they do not feel the ring's presence during sex or in general, and the ring stays in place . Perceived advantages of the method include high rates of efficacy, menstrual cycle control, a nondaily dosing schedule, relatively low drug doses and high user acceptability; disadvantages include changes in vaginal discharge, involuntary ring expulsions, and discomfort during insertion or when engaging in sex .…”

“…However, if the usage of the product increases markedly or waste disposal practices become less rigorous, leaching of EE from discarded monthly rings may become a problem. A vaginal ring delivering 15 μg EE with a potent progestin, Nestorone, has been developed by the Population Council and evaluated in the NICHD CCTN and the International Committee for Contraceptive Research (1417). A single ring can be used cyclically for 13 cycles, having the potential to improve acceptability, accessibility and compliance by requiring fewer refills and only one ring per year to be discarded.…”

“…Results from several studies indicate that the side-effect profile of a contraceptive is fundamental to a woman's decision to either start a method or discontinue use of an existing method [1][2][3]. In the US, approximately 33% of women discontinue use of a reversible contraceptive for method-related reasons within 6 months of starting use, and 46% do so within 12 months [4].…”
