Active Versus Expectant Management of the Third Stage of Labor and Implementation of a Protocol

Abstract: Although the focus at delivery may naturally shift to infant transition, continued maternal vigilance during stage 3 is imperative to accomplish a safe outcome for the mother and her newborn. The third stage of labor is a normal physiological progression of birth that may be compounded by serious complications. The most common complication is postpartum hemorrhage due to uterine atony. Clinicians choose either active management or expectant management for stage 3 to prevent excessive maternal blood loss. Rapid… Show more

“…In natural third stage, the baby's cord is usually not clamped or cut and the mother and baby stay in SSC, in a warm, nonstimulating birthing environment until after the placenta has been delivered. [10][11][12] Directly after the baby has been born, the mother meets her baby for the first time and she experiences a natural behavioral pattern that supports the foundation of confident mothering, early breastfeeding, and a secure bond with her baby. Skin-to skin contact with breastfeeding involves an increase in oxytocin, endorphins and other hormones that promotes maternal child bonding.…”

“…Of postpartum deaths, 45% occur within the first 24 hours and 66% occur within the first week. The prevalence of PPH worldwide is between 6% and 10% [3]. The reason for deaths of PPH is that is not easy to notice immediately or earlier which is important to early diagnosis and intervention for PPH.…”
