Anal Carcinoma: Prognostic Value of Endorectal Ultrasound (ERUS). Results of a Prospective Multicenter Study

Abstract: This muticenter prospective study demonstrated the superiority of ERUS-based staging over traditional clinical staging in the prediction of important outcomes such as local tumor recurrence and patient survival.

“…In the authors' data set this would result in a significant improvement in the overall accuracy of TRUS, yielding a weighted κ value of 0·94. This adjustment may allow TRUS results to provide a better reflection of clinical management decision-making 10,20,21 .…”

“…EAUS has been shown to be superior to MRI in the detection of small superficial tumours of the anal canal [16]. Staging systems on EAUS have been suggested [13,21] which emphasise depth of invasion over size of tumour (Table 3), similar to the staging classification in rectal carcinoma. This staging system is not as widely used as the AJCC staging system.…”

“…13 Progression of disease following chemoradiation on MRI: a Axial T2-weighted MRI pre-treatment demonstrate a 4-cm-long anal canal tumour involving the left puborectalis (white arrow); b Axial T2-weighted MRI taken 7 months later postchemoradiation shows resistance to treatment with disease progression-the mass has further extended through the left puborectalis muscle (white arrow). There is a new right inguinal nodal metastasis (black arrow) sufficient time frame to allow for resolution of oedema and for accurate ultrasound imaging [13]. Serial follow-up examination with EAUS can monitor changes in the size of residual scar tissue and detect local recurrence [23].…”

“…De igual forma, la importante frecuencia de indicación en pacientes con neoplasia maligna rectal confirmada habla de la aceptación a nivel nacional y regional que se le está dando a este diagnóstico por parte de grupos interdisciplinarios de oncología, donde la endosonografía juega un papel fundamental a la hora de complementar otras ayudas diagnósticas para estadificación locorregional y planeamiento adecuado de la terapia (4,9,(19)(20)(21)(22).…”
