Assessment of histamine release from basophils in whole blood by benzylpenieilloyl poly‐L‐lysine in penicillin‐sensitized patients

Abstract: Histamine release from basophil granulocytes in whole blood by benzylpenicilloyl poly-L-lysine (PPL) was investigated in 7 patients with penicillin allergy. All patients presented with systemic immediate hypersensitivity reactions after i.v. administration of penicillin G. Total histamine (of 7 patients) ranged from 27.5 ng/ml to 62.1 ng/ml (mean 43.2 ng/ml). The spontaneous histamine release ranged from 0.15% to 5.1% (mean 1.8%) of the total content. Addition of PPL in various concentrations resulted in value… Show more

“…In a position paper eight studies on a total of 146 patients with a history of immediate reactions to b-lactams and positive skin tests were reviewed showing a sensitivity of 46% (range 35-80%) and a specificity of 79-89% [18]. Similar results were obtained in another more recent study [19]. Combining BAT with sulphidoleukotriene tests increased sensitivity [7], and also the addition of determination of specific IgE-antibodies in serum.…”

“…Results were expressed as a percentage of total histamine content after correction for spontaneous release, and the net histamine release was calculated as follows: (anti‐IgE‐ or drug‐induced release – spontaneous release)/(total histamine content – spontaneous release). The limit of positivity was set at 5%, as previously accepted (17).…”

“…Findings in this study suggest that the TRT and the CAST may even represent an attractive and supplementary mean for diagnostic intervention in food and drug allergy. In fact, much effort has been put into establishing new test in order to improve the diagnosis of food and drug allergy [ 20, 21].…”
