Assessment of Right Ventricular Size and Function: Echo Versus Magnetic Resonance Imaging

Abstract: The usefulness of the echocardiographic "eyeball" method to estimate RV size and systolic function in patients with right heart disease has limitations when compared with MRI, specifically in regard to the variability between echocardiographers.

“…39 Subjective assessment of RV function is common practice, but compared with MRI, its accuracy is low and inconsistent. 40,41 Because of its ubiquity and ease of use, however, subjective assessment of RV function continues to play a central role in PH diagnosis and management. Quantification of RV hypertrophy (RVH) is also difficult to measure and is largely a qualitative assessment.…”

“…This qualitative assessment or eyeball method is clearly limited when compared with quantitative assessment of systolic function with CMR. 6 As a result, CMR has been considered the best available method for assessing RV function. 7,8 In 2010, guidelines were developed for quantitative echocardiographic assessment of the right heart in adults.…”

“…Another limitation was that subjective assessment is by its nature a qualitative measure of RV function and thus prone to depend on the experience of the echocardiographer [31,32]. Furthermore, quality of the TTE study may vary based on patient body habitus and technician.…”

“…This is performed by reviewing the right ventricular contraction in multiple views including parasternal short axis for septal motion, apical four chamber, and subcostal. Variability of right ventricular subjective assessment, given it is not a quantitative measure, may exist between readers, but remains an important part of the standard echocardiographic assessment of the right ventricle [31,32]. Aspects used in the determination of the visual assessment of right ventricular function are summarized in recent guidelines [32].…”
