Association between COMT Polymorphism Val158Met and Opioid Consumption in Patients with Postoperative Pain: A Meta-Analysis

Abstract: Background/Aims: Several factors influencing postoperative pain and the effect of opioid analgesics have been investigated on an individual level. The aim of this study was to clarify the impact of catecholamine-O-methyltransferase (COMT) gene Val158Met on opioid consumption in postoperative patients. Methods: A systematic review and meta-analysis of the literature up to September 30, 2017, were performed by using PubMed, Cochrane Library, ISI Web of Science, and Chinese National Knowledge Infrastructure (CNKI… Show more

“…The catecholamine‐ O ‐methyltransferase ( COMT ) rs4680 Val158Met polymorphism is one of the best‐known single‐nucleotide polymorphisms (SNPs) associated with pain expression . A recent meta‐analysis indicates that patients with the Val/Met allele but not Met/Met allele consume less opioids after surgical treatment . The Met/Met genotype of COMT produces an enzyme defective at metabolizing biogenic amines such as dopamine, thus resulting in variation in responses to pain expression .…”

“…13 The Met/Met genotype of COMT produces an enzyme defective at metabolizing biogenic amines such as dopamine, thus resulting in variation in responses to pain expression. 13 There are other dopamine-related genes whose products alter the levels of dopamine in peripheral blood or cerebrospinal fluid that may alter pain expression. For example, Jonsson et al 14 have reported that specific dopamine receptor D2 (DRD2) genotypes participate in the regulation of monoamine turnover such as homovanillic acid in cerebrospinal fluid.…”
