Behavioral Disorders in Alzheimer Disease: A Transcultural Perspective

Abstract: These results indicate that NPI is a reliable instrument with which to study transcultural differences in the presentation of neuropsychiatric disturbances in patients with AD. The described similar pattern of behaviors between Italians and US patients with AD suggests a biological origin of the disorders. However, cultural influences must be taken in account when the focus of the study is on psychopathological aspects of dementia.

“…A study of an instrument aimed to measure behaviour in AD patients has proved the transnational validity of this instrument, and has shown a similar behavioural pattern between the two different linguistic groups (Italian and USA) examined [4,49]. In conclusion, the clinical features of Alzheimer's disease do not change across different ethnical and cultural settings, suggesting that it is a disease biologically determined and not influenced by cultural or linguistic environment.…”

“…Nevertheless, whereas most of the previous studies were conducted on less homogeneous clinical samples including subjects with possible and probable AD [25,43] or even different forms of dementia [8,15,17], we have assessed a group of subjects with probable AD, thus providing a higher biological validity. The behavioral assessment scale we used (NPI) could be considered quite rough for the evaluation of fine behavioral changes; nevertheless, it is broadly used and well-validated in Italian [33] and could provide information about a wide range of neuropsychiatric symptoms.…”

“…Each subject underwent the 12-item version of the Neuropsychiatric Inventory (NPI) [33,34], a caregiver-based clinical interview exploring 12 behavioral domains (delusions, HALL, agitation/aggression, depression, anxiety, euphoria, apathy, disinhibition, irritability, abnormal motor behavior, sleep, and appetite disturbances).…”

“…The manifestations of BPSD symptoms can vary to some degree according the patients's ethnic, cultural and social background (24)(25). Apathy and depression is reported more in western cultures (24)(25) in contrast to more activity disturbance and aggressiveness reported in patients from non-western backgrounds.…”
