Benefit of antiretroviral therapy on survival of human immunodeficiency virus-infected patients admitted to an intensive care unit

Abstract: Objective To evaluate the impact of antiretroviral therapy (ART) and the prognostic factors for in-intensive care unit (ICU) and 6-month mortality in human immunodeficiency virus (HIV)-infected patients. Design A retrospective cohort study was conducted in patients admitted to the ICU from 1996 through 2006. The follow-up period extended for 6 months after ICU admission. Setting The ICU of a tertiary-care teaching hospital at the Universidade de São Paulo, Brazil. Participants A total of 278 HIV-infected… Show more

“…A continual shift occurred toward ICU admissions for non-AIDS-related diagnoses [4]. Among them, sepsis has been responsible for about 12% to 31% of HIV-infected patient admissions to the ICU, and is associated with a worse prognosis [11,36,37]. In our study, we found that HIV-infected patients had a higher frequency of opportunistic infections than did patients in the control group; despite being in the late period of cART.…”

“…However, specific HIV characteristics (CD4 cell count, plasma HIV-RNA load, HIV-related diagnosis, or antiretroviral therapy) have not been clearly identified as predictors of ICU mortality [4,15,37,38,[49][50][51][52][54][55][56][57][58][59], although some reports have associated CD4…”

“…For example, serum albumin is an independent marker of morbidity and mortality in patients with acute acquired immunodeficiency syndrome (AIDS) admitted to the ICU. (21) In this population, the serum albumin level is a marker for the nutritional deficiency that may influence prognosis. However, the effect of albumin therapy in certain AIDS-associated conditions, such as acute respiratory failure and sepsis, has not been evaluated.…”
