Up to 3 out of every 1000 patients may develop an atrial myxoma, the most unusual benign primary heart tumor. The morbidity and death of patients are signi cantly impacted by these malignancies. Myxoma is the least common primary cardiac neoplasm. Between 0.001 0% and 0.3 0% of autopsies reveal heart malignancies. Even when the ischemic stroke is uncommon, it is still important to consider the probable pattern of left atrial myxoma disclosure while making a stroke differential diagnosis. Case presentation:A case of rapidly growing left atrial myxoma in a 60-year-old female who was suffering from a stroke 15 days ago, she started complaining of breathlessness and chest tightness and incidentally going through transthoracic echocardiography reveals that a large left atrial myxoma. she transferred to cardiovascular thoracic surgery department for further treatment. after all investigation, they planned for open-heart surgery to remove of LA mass. Conclusion:The superior portion of the heart can develop an atrial myxoma, a benign growth, on either the left or right side. Atrial myxoma can have several negative effects if untreated, including irregular heartbeat, pulmonary edema, and blood clots termed emboli (embolisms) that block the heart's blood vessels and impede blood ow. A targeted functional impairment is brutally imposed as part of an ischemic stroke. The prompt surgical removal of a rapidly expanding myxoma that is probably a malignant tumor may be necessary. They may show up as a variety of neurologic conditions, such as ischemic stroke. The most common neurologic symptom of heart malignancies is ischemic stroke.

Acute ischemic stroke (AIS) is a common complication of atrial myxoma (AM), and most emboli originate from a thrombus attached to the surface of the myxoma, with occasional shedding of tumor fragments leading to stroke. Clinical manifestations of AM include mitral valve obstruction, systemic embolism, and weakness. However, neurological deficits and other nonspecific manifestations may mask the presence of AM. The surgical resection is the most effective treatment for these conditions. However, the main problem is the lack of consensus regarding the prevention and treatment of stroke complicated by myxoma during the perioperative period. This study aims to improve the diagnosis and treatment of AM and the associated AIS. Here, we report the case of a 27-year-old patient with AM who presented with chest pain, palpitations, and sudden dizziness and had a stroke under anticoagulant treatment during the admission period. We also reviewed and summarized the clinical presentations and characteristics of similar previously reported cases. Our review emphasized the importance of early diagnosis and appropriate perioperative management of AM and its associated AIS.

Objective: Through this surgical series, we present the epidemiological and anatomical-clinical aspects and the surgical results concerning patients operated on for a primary cardiac tumor at the Abidjan Heart Institute. Materials and Method: This is a retrospective descriptive study covering the period of January 1982 to December 2022, based on the medical records of patients operated on for a primary cardiac tumor at the Abidjan Heart Institute. Results: Twenty-seven (27) patients underwent surgery for a primary cardiac tumor, including 14 women and 13 men with a mean age of 41.5 years (range 19 -76 years). The main circumstances of discovery were exertional dyspnea, palpitation and syncope or pseudo-syncope. The main site was the septal wall of the left atrium. The diagnosis of myxoma was confirmed by pathological examination of the surgical specimen in 96.3% (n = 24) of the patients and it was a malignant large cell immunoblastic lymphoma of the myocardium in 3.7% (n = 1) of the patients. The mean largest diameter was 46.1 mm. The postoperative course was marked by an ischaemic stroke (n = 1); recurrence of a left atrial myxoma 5 years after the first tumor removal (n = 1). Two cases of death were noted, one due to the evolution of immunoblastic large cell lymphoma and the other due to an extracorporeal circulation accident. Conclusion: Almost all primary cardiac tumors operated on in Abidjan are myxomas. The circumstances of the discovery of these cardiac tumors are multiple and varied but dominated by exertional dyspnea, palpitation and syncope. Whatever their histological type, primary cardiac tumors are serious affections, in view of the haemodynamic and rhythmic disorders they cause.
