Episodic (transient/ intermittent) left bundle branch block (LBBB) has been associated with different conditions such as bradycardia, tachycardia, anesthesia, acute pulmonary embolism, changes in intrathoracic pressure, chest trauma, cardiac interventional procedures, mad honey poisoning, and in other clinical settings. Of note, exclusion of an acute coronary syndrome in the setting of episodic LBBB is of great importance. Moreover, episodic LBBB is sometimes symptomatic and may be associated with left ventricular systolic and/or diastolic dysfunction or conduction disturbances leading to syncope. This review article provides a comprehensive overview of the conditions associated with episodic LBBB and discusses the clinical impact of this phenomenon.

Introducción: El bloqueo intermitente de rama del haz de His es un trastorno de la conducción poco común con solo unos pocos casos informados en la literatura, principalmente después de una isquemia miocárdica o un traumatismo cardíaco cerrado o múltiples causas que enumeraremos a lo largo de esta revisión. Presentación del caso: El vasoespasmo coronario como causa principal de bloqueo de rama intermitente ha sido poco informado antes. Presentamos un caso de un hombre de mediana edad con presentación de dolor torácico cardíaco con bloqueo de rama izquierda intermitente en Electrocardiograma. El paciente fue llevado al laboratorio de cateterismo. El angiograma finalmente mostró coronarias con vasoespasmo moderado-severo en la arteria descendente anterior izquierda, que se alivió totalmente tras la colocación de un stent medicado. Discusión y Revisión: El bloqueo de rama intermitente suele presentarse en pacientes con cardiopatía subyacente o enfermedad del sistema de conducción. Hay dos formas de bloqueo de rama aquellos dependientes de la frecuencia: la forma de bloqueo dependiente de la aceleración, en la que se produce un retraso en la conducción cuando la frecuencia cardíaca supera un valor crítico, y el bloqueo dependiente de la desaceleración en el que se produce un retraso en la conducción cuando la frecuencia cardíaca desciende por debajo de un nivel crítico. Conocer este tipo de fenómenos permite una mejor compresión de la fisiopatología de los cambios electrocardiográficos para así abordar oportunamente dicha condición y obtener los mejores resultados posibles.

Electroconvulsive therapy (ECT) remains stigmatized in the broader medical community because of misunderstandings about treatment procedures, mortality rates, and cardiovascular complications. Electroconvulsive therapy causes periprocedural hemodynamic variability because of the surges in parasympathetic and sympathetic nervous systems after the administration of the electrical charge. Patients experience an increase in cardiac workload, which is potentially dangerous for patients with preexisting heart disease. Several findings suggest that cardiac complications occur most frequently in patients with underlying cardiovascular disease. We describe the cardiovascular complications that may result from ECT treatment and offer insight on how to mitigate these concerns if they occur. PubMed was queried using terms "electroconvulsive therapy" and "cardiovascular adverse effects." A table is provided with the common cardiovascular side effects of ECT and the most recent evidence-based treatment strategies to manage them. Generally, ECT is a safe procedure in which complications are minor and manageable. Most major complications caused by ECT are related to the cardiovascular system; however, with an appropriate pre-ECT evaluation and a comprehensive multidisciplinary team approach, the cardiovascular complications can be well managed and minimized. Providing proper cardiac clearance can prevent cardiac complications and provide timely care to treatment-resistant populations who are at risk for excessive morbidity and suicide.
