Case Report: Transient Left Bundle Branch Block Associated with Ect

Abstract: We present the first reported case of transient left bundle branch block (LBBB) occurring during electroconvulsive therapy (ECT). LBBB is an important clinical finding, as it is associated with a significant increase in mortality. Physicians providing ECT should be aware of the significance of new-onset LBBB; it may occur during treatment.

“…Episodic LBBB has been reported in several diverse clinical conditions. These are the following: coronary fistulas connecting main pulmonary artery with coronary arteries, chest trauma, cervical wound, Guillain‐Barre syndrome, takotsubo cardiomyopathy, Graves‐Basedow disease, hemorrhagic stroke, myocarditis in enteric fever, myocarditis along with acute ischemic cerebellar, pontine and lacunar infarction following viper bite, electroconvulsive therapy, endoscopic retrograde cholangiopancreatography, athletic heart syndrome, tachycardia induced cardiomyopathy, endoscopic brachytherapy for lung cancer, and propionic aciduria…”

“…Pending a patient's clinical stability, these findings merit close follow-up with cardiology for an ischemic evaluation 38,39 . Tachycardia after ECT may reveal a “rate-related” bundle branch block 90 . A left bundle branch block may be a sign of a serious underlying condition, such as ischemia or coronary artery disease.…”

A new left bundle branch block with anginal symptoms can represent an MI and warrants immediate evaluation (Table 4).…”
