Clinical diagnosis of Ménière's disease: how useful are the American Academy of Otolaryngology Head and Neck Surgery Committee on Hearing and Equilibrium guidelines?

Abstract: The American Academy of Otolaryngology Head and Neck Surgery guidelines are more sensitive and less specific in diagnosing Ménière's disease than Prosper Ménière's original description. Standardised criteria should be applied across published series, in order to make results accurate, comparable and useful in the long term. We would recommend the introduction of a stricter diagnostic category for definite Ménière's disease.

With the exception for audiometric measurements, the diagnosis of MD is a clinical one, and definitive clinical criteria that satisfy the MD diagnostic guidelines are not always present [Stapleton and Mills, 2008;Hoa et al, 2015]. In addition, patients with mild symptoms may not have been diagnosed, as patients may suffer from variable auditory and/or vestibular symptoms for several years before they meet the diagnostic criteria for MD [da Costa et al, 2002].…”

“…Several comorbidities from the central nervous system, such as depression, sleeping disorders, anxiety, and migraine, were more frequent in cases with MD than in controls. Depression, anxiety, and sleeping problems might occur due to the frightening symptoms of MD; especially vertigo and dizziness have been reported to have a noticeable psychological impact on patients [Stapleton Mills, 2008;Tyrrell et al, 2015]. However, as the recorded index date is somewhat imprecise, we do not know which occurred first, the MD symptoms or the abovementioned diseases.…”

“…A study of Stapleton and Mills [68] about the diagnostics of Menière’s disease only marginally touches the adherence to current guidelines. The authors compared the diagnostic criteria of the AAO-HNS Committee on Hearing and Equilibrium Guidelines with the criteria of the original publication of Prosper Menière from 1861.…”

“…The American Academy of Otolaryngology and Head & Neck Surgery Committee on hearing and equilibrium guidelines define vertigo as 'the sensation of motion when no motion is occurring relative to the earth's gravity' in contrast to motion intolerance which is a feeling of disequilibrium, spatial disorientation or malaise during active or passive movement 1 . Problem Statement: Vertigo is a common symptom especially in the elderly.…”
