Clinical Efficacy and Pharmacokinetics of Levothyroxine Suppository in Patients with Hypothyroidism

Abstract: This study aimed to elucidate the clinical efficacy and pharmacokinetics of levothyroxine (LT4) suppository, thus, we examined the pharmacokinetics of thyroxine (T4) after the administration of the suppository in thyroidectomized rats and examined dose and the levels of free T4 (FT4) in patients with hypothyroidism receiving suppositories. Thyroidectomized rats were administered with LT4 solution and LT4 suppository (30 µg/kg), and plasma T4 concentrations were measured using LC/MS. The AUC 0-168 of T4 after r… Show more

“…Tmax is at about 15 h, similar to when LT4 is given orally, but the area under the curve is about 3 times smaller indicating incomplete absorption. Similar results were obtained in hypothyroid patients (56). Unfortunately, no such studies have been performed with LT3 but should be explored.…”

“…The walls of the rectum are lined with a mucosa that is highly vascularized, allowing for rapid absorption of medications. Formal PK studies after the administration of the LT4 suppository have been performed in thyroidectomized rats (56). Tmax is at about 15 h, similar to when LT4 is given orally, but the area under the curve is about 3 times smaller indicating incomplete absorption.…”

“…Strategies for enhancing compliance, namely supervised oral ingestion and/or once weekly intake of LT4, seem to be an effective and well-tolerated way to overcome this issue. When malabsorption is found, distinct LT4 formulations have been used with success ( 24 , 25 , 26 ). LT4 soft gel and LT4 liquid contain predissolved LT4 and are claimed to improve bioavailability, presumably by facilitating absorption.…”

“…Also, transient hypothyroidism tends to respond to a lower dosage of L-T4 than CH. 7,15,24,25 Kashiwagura et al 26 successfully administered a suppository preparation of L-T4 in patients with hypothyroidism who could not take thyroid hormones by mouth. Because our patient was incapable of receiving oral administration of L-T4 and because intravenous and suppository preparations of L-T4 were not available at our hospital, we decided to empirically administer a rectal diluted solution of L-T4.…”
