Coexistence of primary pulmonary Hodgkin lymphoma and gastric MALT lymphoma associated with Epstein-Barr virus infection: A case report

Abstract: We describe a 66-year-old woman with Epstein-Barr virus-associated lymphoproliferative disorder with lung and gastric tumors. We identified two lung tumors measuring 13 and 20 mm in diameter that consisted of CD30-, CD15-, and CD20-positive Hodgkin- and Reed-Sternberg-like cells and heterogeneous cellular infiltrates in a pronounced nodular pattern, with necrosis and vasculitis, diagnosed as nodular sclerosis classical Hodgkin lymphoma. A gastric tumor showed low-grade extranodal marginal zone B-cell lymphoma … Show more

“…The true contribution of EBV to Hodgkin's lymphoma remains unknown 30. There are only a few reports of PPHL associated with EBV infection 24 26 31. Prior infection with EBV is reported in 33–50% of all cases of Hodgkin's lymphoma 24 30.…”

“…EBV implication in MALT-type lymphoma development remains doubtful and not well understood [15, 16]. Globally, MALT-type lymphoma is infrequently associated with EBV [15, 17]. In our case, we could not conclude that EBV infection is the reason of the MALT-type lymphoma development, since the neoplastic lymphoid population was EBER negative.…”

“…Oka et al reported a patient with synchronous gastric MALT diagnosed at the same time as primary pulmonary Hodgkin lymphoma. Of note, this patient was infected with EBV, a known predisposing risk factor for Hodgkin lymphoma [17]. Both HRS-like and lacunar cells in the lung tumor and centrocyte-like cells in the MALT lymphoma expressed EBER, so it is likely that EBV was involved in the pathogenesis of both.…”
