Background The Baska mask is a supraglottic airway device that has a continuous non-inflatable flexible cuff that mimics an airway, reducing the risk of overinflation and aspiration of gastric contents. This prospective study from a single center aimed to compare the safety and efficacy of the Baska mask vs endotracheal intubation (ET) in 64 women undergoing elective laparoscopic gynecological surgery in Trendelenburg position under general anesthesia. Material/Methods This was a prospective randomized study conducted on 64 adult female patients, 32 in each group, undergoing gynecologic surgery under general anesthesia. The study population was divided into 2 groups: the Baska group and the ET group. The respiratory and haemodynamic variables, the degree of gastric distension, and postoperative pharyngolaryngeal symptoms were compared between groups. Results After 2 patients dropped out of the study, data from 62 patients were analyzed. The Baska group showed significantly shorter device insertion time (28.4±10.7 vs 46.6±19.8, P =0.001), a significantly lower oropharyngeal leak pressure at all studied time points, and a higher leak fraction at 1 min after the completion of pneumoperitoneum (5.6% vs 2.1%, P =0.031) compared to the ET group. Respiratory and haemodynamic variables, the gastric antrum size, and postoperative pharyngolaryngeal symptoms showed no significant difference. Conclusions This study supports the findings from previous studies in patients undergoing general surgery. The Baska mask was as effective as ET in ventilation during general anesthesia in women undergoing elective laparoscopic gynecological surgery. Respiratory and hemodynamic responses were similar to ET, and no complications were associated with the Baska mask in this study.

Background:Use of Baska Mask, since a supraglottic airway device, could result in low incidence of postoperative pharyngolaryngeal complications.Aims:The objectives were to compare the effect of Baska Mask versus endotracheal intubation on the incidence and severity of postoperative sore throat (POST), postoperative hoarseness of voice (POHV), and postoperative cough (POC).Settings and Design:This prospective randomized study was conducted in a tertiary care teaching institute in 120 patients undergoing short laparoscopic surgeries.Subjects and Methods:Airway was secured using with low-pressure, high-volume cuffed endotracheal tube in Group T and with Baska Mask in Group B. POST, POC, and POHV were assessed at 2, 6, 12, and 24 h after surgery. Those with Grade III score throat were managed by gargling with dispersible aspirin 75 mg.Statistical Analysis Used:Pearson's Chi-square test, Fisher's exact test, and independent sample t-test were used as applicable.Results:Group B patients required significantly more attempts at securing airway. Incidence of POST was significantly higher in Group T as compared to that Group B at 2, 6, 12, and 24 h after surgery. POC was significantly more in Group T at 2, 6, and 12 h. No patient in Group B had POC at 12 and 24 h. POHV showed significantly higher incidence in Group T at 2 and 6 h. No patient in Group B had hoarseness at 6, 12, and 24 h.Conclusion:Use of Baska Mask as compared to endotracheal intubation significantly reduces the incidence and severity of pharyngolaryngeal complications such as POST, POC, and POHV in patients undergoing short laparoscopic surgeries.

Проведен анализ литературных данных по особенностям применения ларингеальной маски (ЛМ) в лапароскопической хирургии с целью обеспечения эффективной и безопасной анестезии. ЛМ - это разновидность надгортанных воздуховодных устройств, которые являются альтернативой интубации трахеи и имеют ряд преимуществ с большим профилем безопасности. Возможность и целесообразность использования ЛМ при робот-ассистированных ивидеолапароскопических оперативных вмешательствах остается спорным вопросом в связи с опасениями возникновения осложнений - интраоперационной регургитации и легочной аспирации на фоне повышенного внутрибрюшного давления при искусственно созданном карбоперитонеуме. В данном обзоре приведены преимущества применения ЛМ 2-го поколения при лапароскопии, конструкция которых позволяет обеспечивать эффективную вентиляцию с достижением адекватного дыхательного объема и оксигенации. Однако необходимо соблюдать четкие правила установки ЛМ и контроля за безопасностью их использования при анестезиологическом обеспечении лапароскопических вмешательств. The analysis of literature data on the peculiarities of the use of laryngeal mask (LM) in laparoscopic surgery was carried out in order to ensure effective and safe anesthesia. LM is a type of supraglottic airway devices that is an alternative to tracheal intubation, and it has a number of advantages with a large safety profile. The possibility and feasibility of using LM in robotic-assisted and videoassisted laparoscopic surgical interventions remain controversial due to the fear of complications intraoperative regurgitation and pulmonary aspiration on the background of increased intraabdominal pressure with artificially created carboperitoneum. This review summarizes the advantages of using the 2nd generation LMs in laparoscopy, the design of which lets to provide efficient ventilation with the achievement of adequate tidal volume and oxygenation. However, it is necessary to follow the clear rules for installing LMs and monitoring the safety of their use in the anesthetic provision of laparoscopic interventions.
