Comparison of two automated mononuclear cell collection systems in patients undergoing extracorporeal photopheresis: a prospective crossover equivalence study

Abstract: CMNC and MNC systems offer different advantages in different clinical conditions, and both are safe and efficient in collecting MNCs for ECP.

“…We did not find any correlation between MNC CE2 and MNC precount. These results are in line with those reported by Del Fante et al suggesting that MNC CE2 is stable regardless of patient preprocedure counts . Interestingly, we observed a slight ccorrelation between lymphocyte collected per liter of blood processed and lymphocyte preprocedure counts but only in the Spectra Optia group.…”

“…In our study, even if higher in the Spectra Optia group, product Hct was constantly below 3% and did not affect the illumination time This is also in line with findings reported by Piccirillo et al A recent study reported a higher product Hct with the MNC protocol compared to ghe CMNC program This is consistent with our finding, as most of the Spectra Optia procedures (67%) in our study were run with the MNC protocol. The higher product Hct observed in the Spectra Optia group may also reflect collections deeper in the MNC layer, explaining also a part of the higher MNC collection efficiencies as well as the higher granulocyte contamination.…”

“…More recently, in 2009, the third-generation Therakos Cellex device has become available with advances including continuous-flow separation technology, shorter treatment times (1.5 versus 3 hours) and reduced extracorporeal blood volumes allowing for lower weight patients to safely undergo the procedure. 15 Brosig et al prospectively compared offline versus online systems. Notable differences were seen in the cellular composition of the two methods.…”
