Comparison of viscoelastic coagulation analysis and standard coagulation profiles in critically ill neonatal foals to outcome

Abstract: Viscoelastic coagulation evaluation could be used in a neonatal intensive care unit setting to further characterize coagulopathy, and identify foals at higher risk for poor outcome.

“…The correlation between conventional coagulation tests and sonoclot parameters is rather limited [17, 18]. In this study we have tried to compare the sonoclot parameters with conventional tests so as to be able to replace the need for several coagulation tests in these patients with sonoclot.…”

“…Thromboelastography provides a global assessment of the coagulation steps in real time, from clot formation to fibrinolysis . Reference ranges for the Sonoclot, a viscoelastic coagulation and platelet function analyser, have been reported in foals and adult horses, including both healthy animals and those with different disturbances . Nonetheless, there are no published data concerning thromboelastography in donkeys, using either the Sonoclot or other analysis systems such as rotational thromboelastometry (ROTEM).…”

“…Healthy foals were found to have increased platelet activity compared with adult horses, and an association with positive blood culture status and hypocoagulability (decreased clot rate) was identified in a small group of foals with sepsis 79 . In a slightly larger study evaluating standard coagulation profiles, SCPFA variables, and outcome in foals with presumed sepsis, hypocoagulability (decreased CR) was associated with death, and prolonged ACT (delayed clot formation) as well as hypocoagulability was associated with development of coagulopathy 80 …”

“…These analyzers are currently being evaluated in horses and may be useful in the future for use in clinical cases. Coagulation evaluation of healthy foals, a small number of adults, and foals with suspected sepsis has been explored using the SCPFA 79,80 . Healthy foals were found to have increased platelet activity compared with adult horses, and an association with positive blood culture status and hypocoagulability (decreased clot rate) was identified in a small group of foals with sepsis 79 .…”
