Contribution of the 2-[18F]-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography to the diagnosis of primary osseous Hodgkin lymphoma

Abstract: Hodgkin lymphoma typically presents as a nodal lesion and infrequently involves extra nodal sites. Although cases of primary extra-nodal Hodgkin lymphoma have been reported previously, the reality of the primitive nature of the disease was difficult to authenticate with traditional high resolution imaging techniques, such as computed tomography or magnetic resonance imaging, because they cannot evaluate the spread of the disease throughout the whole body. We report here a case of primary osseous Hodgkin lympho… Show more

“…The recent studies suggest that 2-[18 F]-fluoro-2-deoxy- d -glucose positron emission tomography/computed tomography (18 F-FDG-PET/CT) enables systemic HL with secondary bone invasion to be distinguished from primitive osseous HL. This technique is highly specific in demonstrating the isolated osseous localization of the tumor and should be recommended in all patients with putative osseous lymphoma [ 25 ].…”

“…An additional question remains as to whether multifocal cortical bone disease represents more advanced disease and should, therefore, be staged and treated accordingly. There are no guidelines regarding the use of chemotherapy versus radiotherapy or a combination of the two in cases of isolated or multifocal bony lesions . It does not appear that one treatment modality is superior to another or to a combination of chemotherapy and radiotherapy, as all the case reports reviewed reported favorable outcomes regardless of the treatment chosen.…”

“…The most frequently used regimen (n = 10) was adriamycin, bleomycin, vinblastine, and dacarbazine (ABVD). 6,9,13,31,33,34,37,38 Ten patients received chemotherapy alone, 7,9,13,29,31,33,34,37,38,43 whereas six received combined modality treatment with chemotherapy and radiation. 6,13,15,32,33 One report did not mention the treatment used, nor did it comment on survival.…”

“…5) diagnosed on FDG PET or PET/CT that show intense FDG uptake. [24][25][26] The prognosis depends on the response to systemic chemotherapy, and proper evaluation for treatment effectiveness is essential. FDG PET/CT has been reported to be superior to CT and MRI for the evaluation of response to therapy, and it plays a major role in decision making about further management of primary bone lymphoma.…”
