Rigorous control of water quality in hemodialysis services is extremely important in order to guarantee a better quality of life of the patients submitted to this treatment. The lack of adequate water monitoring has caused the death of various patients in the past. The objective of the present study was to determine the physicochemical and bacteriological characteristics of water used by hemodialysis services in hospitals of the city of São Luís, Maranhão, Brazil. Bacteriological analyses included the membrane filter method for the determination of total coliform bacteria, the Cult-Dipcombi-TTC-agar method for heterotrophic bacteria and the limulus amebocyte lysate method for the determination of endotoxins. Eighteen water samples obtained from three hospital units, six samples per hemodialysis service, collected directly at the pre-and posttreatment points, were analyzed. Microorganisms were detected in the water used by the hemodialysis services in two of the three hospital units (B and C) studied. No contamination with heterotrophic bacteria was observed in pretreatment samples, while endotoxin production was detected in 100% of the samples. In post-treatment samples, heterotrophic bacteria were detected in 66.6% of the samples and endotoxins in 33.3%. The microorganisms identified in unit B were Burkholderia cepacia, Alcaligenes xylosoxidans, Pseudomonas aeruginosa and Stenotrophomonas maltophilia. In unit C Flavimonas oryzihabitans, Ralstonia pickettii and Burkholderia cepacia were identified. A significant correlation was observed between the presence of endotoxins and the physicochemical characteristics of water such as turbidity and conductivity. These data indicate that two of the three hospital units studied should revise the control of their hemodialysis water system.

A experiência da doença é a maneira pela qual os indivíduos respondem à doença, atribuindo significados e buscando maneiras para lidar com ela no seu cotidiano. O estudo teve como objetivo compreender a experiência da Fibrose Cística a partir do contexto familiar. Estudo de caso etnográfico, realizado com famílias de crianças com a doença atendidas em um hospital escola do interior do estado de São Paulo. Os resultados foram divididos nos temas: passado, presente e futuro e permearam essas fases a busca pelo significado da doença e pelo suporte social, a importância da religião e espiritualidade e a centralidade da socialização da criança. Conhecer a experiência na doença e a rede social torna-se imprescindí-vel para o planejamento de um cuidado integral. Essa abordagem mostrou-se inovadora para o cuidado à doença crônica. DESCRITORES ABSTRACTCystic Fibrosis is a chronic disease with an extensive impact on family life. The experience of disease is the manner how individuals respond to the disease, assigning meaning meanings and searching for ways to deal with it in their daily lives. The objective of this study was to understand the experience of Cystic Fibrosis in the family context. This is an ethnographic study, performed with families of children assisted at a teaching hospital located in the state of São Paulo. Results were divided into the themes past, present and future, and all phases were permeated with the search for the meaning of the disease and social support, the importance of religion and spirituality, and child's socialization. Knowing the experience in the disease and the social network is indispensible when planning comprehensive care. This is an innovative approach in health care for chronic diseases. El estudio objetivó comprender la experiencia de Fibrosis Quística a partir del contexto familiar. Estudio de caso etnográfico, realizado con familias de niños con la enfermedad atendidos en hospital escuela del interior de San Pablo. Los resultados se dividieron en los temas: pasado, presente y futuro y atravesaron esas fases la búsqueda del significado de la dolencia y el apoyo social, la importancia de la religión y espiritualidad y la centralidad de socialización del niño. Conocer la experiencia de la enfermedad y la red social es imprescindible para el planeamiento de un cuidado integral. Este abordaje se mostró innovador para el cuidado de la enfermedad crónica.

Perfil nutricional, clínico e socioeconômico de pacientes com fibrose cística atendidos em um centro de referência no nordeste do Brasil

OBJETIVO: Descrever o perfil de pacientes portadores de fibrose cística (FC). MÉTODOS: Estudo transversal, prospectivo, avaliando fibrocísticos de < 18 anos, durante o período de março a julho de 2006, em um centro de referência no nordeste do Brasil. A avaliação nutricional foi realizada pelo escore Z de altura/idade (A/I), peso/idade (P/I) e peso/altura (P/A) e %peso/altura (%P/A), além de medidas de composição corporal. Foram obtidos dados socioeconômicos e clínicos. RESULTADOS: Foram avaliados 21 pacientes, sendo 12 (57,1%) do sexo feminino. A média de idade de diagnóstico foi de 3,8 ± 3,9 anos, e as principais características ao diagnóstico foram infecção respiratória (85,7%), esteatorreia (66,7%) e déficit nutricional (47,6%). A média de escore Z para P/I, A/I e P/A, respectivamente, foi de -0,73 ± 0,28, -0,34 ± 0,21 e -0,73 ± 0,35. A média de %P/A foi de 94,52 ± 1,58. O percentual de desnutridos divergiu quando avaliado pelo escore Z e %P/A (déficit nutricional em 66,7% e 33,3%, respectivamente; p > 0,05). Os pacientes eutróficos apresentaram melhores condições socioeconômicas (p > 0,05) e clínicas, com melhor escore de Shwachman (p < 0,05) quando comparados aos distróficos. CONCLUSÕES: Eutrofia foi encontrada através dos indicadores nutricionais (P/I, A/I e P/A), ao passo que déficit nutricional foi encontrado quando avaliada a composição corporal. As condições socioeconômicas apresentaram-se favoráveis, principalmente em relação ao grau de instrução materna e renda per capita. A idade de diagnóstico foi maior do que o relatado na literatura, embora o escore de Shwachman e o número de infecções respiratórias tenham demonstrado que os pacientes tinham boas condições clínicas.
