Cost analysis of transanal endoscopic microsurgery for rectal tumours

Abstract: This study has shown that TEM is a safe and extremely cost-effective approach for excision of selected rectal tumours including rectal adenomas and early well differentiated rectal cancers (pTis & pT1).

“…In long-term followup, the modality of TEM excision of rectal tumors has proven to be safe and effective, with morbidity and mortality similar to that of conventional transanal excision [6-10]. However, although TEM has been in use for more than 20 years, it has been slow to become universally adopted by colorectal surgeons, in part because of a steep learning curve, but also because of the significant cost of the highly specialized instrumentation [11,12].Technology continues to undergo rapid evolution. In parallel, the surgeon's minimally invasive surgery skill has matched this advancement.…”

“…Nevertheless, it has been proven to be cost-effective when compared to open resection in high volume centers, mainly driven by the shorter hospital stay. [5][6][7] The second rigid platform is the transanal endoscopic operation (TEO; Karl Storz, Tuttlingen, Germany). This less-expensive alternative allows surgeons to use standard laparoscopic instruments, including a 5 mm 30° laparoscope and overall laparoscopy setup, making this platform more affordable, and may be more attractive for surgeons with prior laparoscopic experience.…”
