CSF overdrainage in shunted intracranial arachnoid cysts: a series and review

Abstract: Posterior fossa overcrowding and acquired Chiari I malformation can develop after excessive CSF drainage of intracranial ACs. Overshunting manifestations require prompt recognition and management. Preventive measures consist of making a stringent selection of cases being considered for surgery, avoiding CP drainage, and placing of a programmable valve as initial treatment of intracranial ACs if shunting is considered.

“…But CSF hypotension seems to be the triggering factor in overdrainage. 18 Under low ICP, the venous system becomes distended, leading to meningeal swelling, which in turn contributes to dural and suture fibrosis. The low ICP also leads to a deceleration in cranial growth and ultimately craniocerebral disproportion.…”

“…Unfortunately, shunt placement carries the additional risk of shunt dependency and overdrainage. 40,49,69 We believe that subdural shunt placement, a treatment that has been sometimes advocated for these lesions, 3,28 will usually result in the unintended effect of maintaining a fistula though this torn lining, thus leading to a persistent tear and shunt dependency. 33,85 Acetazolamide has occasionally been used to treat symptomatic hygromas associated with arachnoid cysts.…”
