Current Progress in the Endogenous Repair of Intervertebral Disk Degeneration Based on Progenitor Cells

Abstract: Intervertebral disk (IVD) degeneration is one of the most common musculoskeletal disease. Current clinical treatment paradigms for IVD degeneration cannot completely restore the structural and biomechanical functions of the IVD. Bio-therapeutic techniques focused on progenitor/stem cells, especially IVD progenitor cells, provide promising options for the treatment of IVD degeneration. Endogenous repair is an important self-repair mechanism in IVD that can allow the IVD to maintain a long-term homeostasis. The … Show more

“…Another possibility is that after multiple rounds of differentiation and proliferation, the self-renewal ability of IVD stem cells becomes exhausted in response to degeneration-related or age-related damage. The possible reason includes nutrition deprivation, hypoxia, less reactive oxygen species, inflammatory factors, increased extracellular matrix (ECM) stiffness, and cyclic tension ( Li et al, 2014 ; Zhang et al, 2020b ; Zehra et al, 2022 ).…”

“…A primary concern with stem cell-based therapies is with the potential loss of cells and diminished viability upon administration [ 28 ]. Due to the biological and ethical concerns that arise with the use of stem cells, it would be difficult to justify repeated implantation, which could potentially be necessary if challenges associated with administration cannot be adequately addressed [ 23 ]. Furthermore, the transplantation of exogenous cells is expensive.…”

“…There has been progress with these studies, but there are concerns about a low survival rate of transplanted cells, which would make it difficult for researchers to control for cell viability and differentiation, and the risk of mutation [ 21 , 22 ]. With embryonic stem cells, there is also a potential lack of institutional readiness and societal acceptance [ 23 ].…”

“…In addition to pain, LBP restricts physical activity and can lead to disabilities. Additionally, it causes a major socioeconomic burden [ 1 4 ]. Clinically, LBP always positively correlates with the degeneration of the intervertebral disc (IVD) [ 2 , 5 ].…”
