Diagnosing and Treating Cytomegalovirus Pneumonia in Patients with AIDS

Abstract: Because cytomegalovirus (CMV) can be isolated from pulmonary secretions of human immunodeficiency virus (HIV)-infected patients without causing disease, its clinical significance as a cause of pneumonia in this patient population is frequently questioned. In a 22-month period, CMV pneumonia was diagnosed in 17 (8%) of 210 HIV-infected patients who underwent lung biopsy on the basis of microbiological and histologic criteria. The clinical presentations of these patients were nonspecific, including fever (100% o… Show more

“…CMV pneumonia is uncommon and has been observed in only 0.24-8% of HIV-infected patients [9,10]. The diagnosis of CMV pneumonia should take into account pulmonary interstitial infiltrates, identification of multiple CMV inclusion bodies in lung tissue, and the absence of other pathogens that are more commonly associated with pneumonia occurring in this population [10].…”

CMV hepatitis is also uncommon in HIV-infected patients, although it does occur frequently in liver transplant recipients [11].…”

“…En concreto la afectación pulmonar tiene dificultades aña-didas al diagnóstico ya que el aislamiento en LBA no diferencia la infección asintomática de la neumonitis, es frecuente la coexistencia con otros patógenos y el patrón radiológico no es específico (2,5). Tampoco existe clara relación entre el cultivo de CMV en sangre y en LBA (6).…”

“…The most common manifestations of PCP are subacute onset of progressive dyspnea, fever, non-productive cough, chest discomfort and respiratory failure (1). While cytomegalovirus (CMV) infections with retinitis and gastrointestinal disease were most often observed in patients with HIV, CMV pneumonitis was rarely emphasized (2,3). Negligence of CMV pneumonia could lead to serious consequences (4).…”
