Differential Association Between Statin Exposure and Elevated Levels of Creatine Kinase

Abstract: Compared with simvastatin, lovastatin was generally associated with a lower prevalence of high elevation and mild to moderate elevation of CK levels. An elevated SCr level, exposure to interacting drugs, male gender, evidence of diabetes, and age < or =65 years were associated with higher prevalence ratios.

“…The SAS System version 9.2 (SAS Institute, Inc., Cary, North Carolina, USA) was used for all statistical analyses. The data were reanalyzed after excluding persons who were using statins, because these drugs may cause an increase in creatine kinase activity [17]. We also repeated the analysis after excluding those with uncontrolled hypertension, since the association between creatine kinase and blood pressure might have been diluted if antihypertensive drugs were taken, and we excluded those with impaired renal function since this condition is associated with higher creatine kinase activity [18].…”

“…A cross-sectional study [26] of prescriptions and laboratory data for 215 191 patients exposed to a statin (either lovastatin or fluvastatin) in 2002 investigated the prevalence of high creatinine kinase levels (regardless of presence or absence of muscle-related complaints) and the factors associated with these elevations. Prevalences of high (!1500 I/U) and mild-to-moderate creatinine kinase elevation (!500 U/l and <1500 U/l) were a mean of 1.6 and 6.4 episodes per 1000, respectively.…”
