Background: Epilepsy is a common chronic neurological disorder with more than half of cases beginning in childhood. Most patients require long-term, and sometimes life-long, therapy with antiepileptic drugs (AEDs). AEDs are associated with significant side effects including radiological evidence of rickets, decreased bone mineral density (BMD), altered bone turnover, and increased risk of fracture. Aim of the Study: To assess the bone mineral density in epileptic children getting long term anti-epileptic drugs. Materials and Methods: This study was conducted in the Saleh Child Development and Disability Management Center (SCDDMC), of the Institute of Child and Mother Health (ICMH) Matuail, Dhaka. A total of 31 childrens age ranged 5-15 years diagnosed as epilepsy on the basis of both clinical examination and investigation (EEG) receiving antiepileptic drugs (AEDS) for at least two years were recruited in this study. Results: Almost two third (61.3%) children age group were in 5 to 10 years and 12(38.7%) in 10 to 15 years. Around three fourth (74.2%) children were male and 8(25.8%) were female. More than two third (67.7%) children had adequate sun exposure and more than three fourth (77.4%) children got adequate calcium and vit.D rich food. Majority (83.9%) of the patients got phenobarbitone (PB) followed by 17(54.8%) got combination, 13(41.9%) got valproic acid (VPA), 11(35.5%) got Nitrazipum (NTZ). More than one third (38.7%) children got mono therapy, 19(61.3%) got polytherapy. Majority of the children (96.8%) had serum calcium normal. The mean serum calcium was 9.22±0.78 mg/dl. The mean BMD of spine was 0.66±0.14. The mean BMD of neck left femur was 0.66±0.15. The mean BMD of neck right femur 0.66±0.15. More than two third (67.7%) children had low (<-1SD) BMD Z score. Almost two third (60.0%) children had mono therapy showed BMD Z score normal (>-1SD) and 6(28.6%) had BMD Z score low (<-1SD). Almost three fourth (71.4%) children got poly therapy showed low BMD and 4(20.0%) got poly therapy showed normal BMD Z score. No significant association was found between BMD score and type and duration of therapy. Conclusion: AEDs decreased bone mineral density due to long term use as mono-therapy or polytherapy. The assessment of BMD among children with epilepsy receiving long term AEDs is essential.

Background Epileptic children can pose an additional risk of poor bone health; this study aimed to evaluate the influence of anti-seizure medications (ASMs) on vitamin D level and sclerostin as a bone turnover biomarker in children with epilepsy. Subject and methods This case–control comparative study was conducted on 180 children aged from 5–18 years diagnosed with epilepsy according to the definition of the International League Against Epilepsy on ASMs for more than 3 months and were classified into 90 epileptic children on ASM monotherapy and 90 epileptic children on ASM polytherapy, in addition to 90 healthy children age- and sex-matched who served as controls. After obtaining basic data, laboratory investigations were performed, including serum calcium, phosphorus, alkaline phosphatase, parathyroid hormone, vitamin D, and serum sclerostin. Results When we compared epileptic patients to the control group, there was a statistically significant low level of vitamin D, calcium, and phosphorus and a high level of sclerostin among both epileptic groups with mono or polytherapy. Sclerostin has a statistically significant negative correlation with vitamin D, alkaline phosphatase and parathyroid hormone. Additionally, it has a negative correlation with serum phosphorus, but without a significant correlation. On the other hand, sclerostin has a statistically positive correlation with age and serum calcium, but without a significant correlation. Multiple linear regression analyses were conducted to predict the contributing factors of sclerostin. Only duration of treatment and BMI were significant predictors of high levels of sclerostin. In contrast, the other factors failed to show any significant contribution. Conclusion The present study showed that ASMs modulate the serum levels of sclerostin and vitamin D hence, might be involved in their adverse effects on bone.

TÓM TẮT Đặt vấn đề: Mặc dù có nhiều lựa chọn điều trị thay thế cho bệnh động kinh, bao gồm kích thích dây thần kinh phế vị (VNS), phẫu thuật và chế độ ăn ketogenic, thuốc chống động kinh (AED) luôn là lựa chọn hàng đầu vì dễ áp dụng. Việc điều trị thuốc đôi khi kéo dài và có thể cần liều lượng lớn cũng như phối hợp thuốc. Do đó, ảnh hưởng bất lợi của thuốc chống động kinh nên được xem xét trong quá trình điều trị. Một trong những tác động như vậy là đối với sự chuyển hóa canxi. Vì vậy cần đánh giá tác động lên nồng độ canxi cũng như tăng trưởng của trẻ. Đối tượng, phương pháp: Một nghiên cứu tiến cứu được tiến hành trên 78 trường hợp bệnh nhi được chẩn đoán và điều trị động kinh tại Trung tâm Nhi khoa Bệnh viện Trung ương Huế. Kết quả: Nồng độ Canxi máu toàn phần ở nhóm có điều trị Carbamazepine (CBZ) theo nhóm tuổi (0 - < 2 tuổi, 2 - < 6 tuổi và 6 - < 16 tuổi) sau 9 tháng điều trị lần lượt là 2,38 ± 0,02 mmol/l; 2,31 ± 0,02 mmol/l, 2,27 ± 0,01 mmol/l; các trị số giảm so với tham số bình thường sau 6 tháng điều trị, giảm so với nhóm chỉ điều trị valproate acid (VPA) sau 3 tháng điều trị. Sự khác biệt có ý nghĩa thống kê p < 0,05. Kết luận: Nên tiến hành xét nghiệm sàng lọc canxi, đánh giá tăng trưởng đặc biệt là chiều cao ít nhất mỗi 3 tháng điều trị, đặc biệt với nhóm điều trị CBZ. ABSTRACT The impact of Anti - Epileptic drugs on children’s blood Calcium concentrations Background: Although there are many alternative treatment options for epilepsy, including vagus nerve stimulation (VNS), surgery, and the ketogenic diet, antiepileptic drugs (AEDs) are always the first choice because they are easy to apply. Drug treatment is sometimes prolonged and may require large doses and drug combinations. Therefore, the adverse effects of antiepileptic drugs should be considered during treatment. One such effect is on calcium metabolism. Therefore, evaluating the impact on calcium concentration and child growth is necessary. Methods: A prospective cohort study was conducted on 78 cases of patients diagnosed and treated for epilepsy at the Pediatric Center of Hue Central Hospital Results: Whole blood Calcium concentrations in the Carbamazepine (CBZ) treatment group according to age group (0 - < 2 years old, 2 - < 6 years old and 6 - < 16 years old) after 9 months of treatment were 2.38 ± 0.02 mmol/l; 2.31±0.02 mmol/l, 2.27 ± 0.01 mmol/l; The values decreased compared to normal parameters after 6 months of treatment, decreased compared to the group treated with only valproate acid (VPA) after 3 months of treatment. The difference is statistically significant p < 0.05. 'Conclusions: Calcium screening test and growth assessment, especially height, should be performed at least every 3 months of treatment, especially in the CBZ treatment group.
