Efficacy of topical photodynamic therapy for keratoacanthomas: A case-series of four patients

Abstract: Topical photodynamic therapy (PDT) is an excellent treatment option for various non-melanoma skin cancers and precancerous lesions, including actinic keratosis, Bowen's disease, and basal cell carcinoma. The clinical use of PDT includes a broad range of neoplastic, inflammatory, and infectious skin diseases. There is also anecdotal evidence suggesting the efficacy of PDT for the treatment of keratoacanthomas (KA). We report a case-series of four patients with solitary KA confirmed by histology, treated with to… Show more

“…The data regarding the use of PDT for keratoacanthomas is however scarce and limited to case presentations or case series. While some authors show that PDT could be a good alternative for patients with keratoacanthomas (100,101), other authors suggest that keratoacanthomas could develop after PDT (102,103). Further research is therefore mandatory to support the usefulness of PDT for the treatment of keratoacanthomas.…”

“…35 All four lesions disappeared completely, with no recurrence at 3 years and excellent cosmetic outcomes. 35 These cases suggest that MAL-PDT may be a successful treatment option for SCC, but PDT in general has limited use for the treatment of invasive SCC. With the risk of metastatic disease, surgical intervention should remain the first-line option, with PDT or other therapies used for patients in whom surgery is not an option.…”

“…There are several reports showing that red light PDT, using either 5-aminolevulinic acid or methyl-aminolevulinate as the prodrug, can treat solitary 7 or multiple KAs 3 , 4 with reasonable efficacy. However, our case appears to be the first report, to our knowledge, of PDT specifically used in the Grzybowski variant of KA.…”

“…As a safe, low-cost alternative to systemic agents for ameliorating the worst lesions, we decided to try photodynamic therapy (PDT) because of its reported efficacy in spontaneous KA. 3 , 4 Red light (635 nm Aktilite, Galderma Laboratories, LP, Forth Worth, TX) at a dose of 60 to 80 J/cm 2 was delivered 2.5 hours after application of 16.8% methyl aminolevulinate cream to her forearms and hands. To minimize severe pain, no occlusion was used.…”
