Introduction: Tooth autotransplantation is the repositioning of an erupted, partially erupted, or non-erupted autologous tooth from one site to another within the same individual. Several factors influence the success rate of the autotransplant, such as the stage of root development, the morphology of the tooth, the surgical procedure selected, the extraoral time, the shape of the recipient socket, the vascularity of the recipient bed, and the vitality of the cells of the periodontal ligament. The aim of this scoping review was to provide the most up-to-date information and data on the clinical principles of the third-molar autograft and thus provide clinical considerations for its success. Materials and methods: This review was conducted based on PRISMA-ScR (Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews). The research was conducted by searching for keywords in three databases—PubMed, Scopus and Google Scholar—by two independent reviewers following the PRISMA protocol, from which 599 records were identified. Conclusions: Third-molar autotransplantation is a valid solution to replace missing teeth. The key to the success of this technique is the surgical procedure, which must be as atraumatic as possible to preserve the periodontal ligament of the tooth to be transplanted. The success rate is also linked to the stage of development of the root, with a worse prognosis in the case of a complete root.

Even if a recent case report has demonstrated a positive 5‐year outcome for an autotransplanted tooth in which extraoral root‐end resection and filling were performed without any conventional orthograde treatment (Boschini et al 2020), an early root canal treatment after the surgical procedures may increase the success rate of the intentionally replanted teeth even when apicoectomy and retrograde filling are performed extraorally (Chung et al 2014).…”

Surgical extrusion is defined as the procedure in which the remaining tooth structure is repositioned at a more coronal/supragingival position in the same socket in which the tooth was located originally. Intentional replantation is defined as the deliberate extraction of a tooth and after evaluation of root surfaces, endodontic manipulation and repair, placement of the tooth back into its original position. Tooth autotransplantation is defined as the transplantation of an unerupted or erupted tooth in the same individual, from one site to another extraction site or a new surgically prepared socket. The advent of titanium implant rehabilitation has reduced the use of these treatments in day‐by‐day clinical practice; however, the re‐emerging trend to conserve and preserve natural sound tissues has led to a rediscovery of these treatments. All three distinct surgical methods are closely related, as they act to treat teeth that cannot be predictably treated using other more conventional procedures in endodontics, periodontics and restorative dentistry. Furthermore, these procedures share the same treatment approach and include the atraumatic extraction of a tooth, visual inspection of the tooth/root and its subsequent replantation. The clinical procedures for surgical extrusion, intentional replantation and tooth autotransplantation treatment have undergone several changes in recent years, and currently, there are no clear clinical treatment protocols/guidelines available. The clinician should be aware of the outcome of these treatments. Hence, the aim of this narrative review is to provide the background, clinical procedures and outcomes of surgical extrusion, intentional replantation and tooth autotransplantation.

Introdução: O transplante dentário consiste numa técnica de transferência do dente de um alvéolo ao outro na maxila ou mandíbula, sendo uma boa opção terapêutica para pessoas que perderam o dente de forma precoce. Para a prática clínica o transplante de interesse é o autógeno, ou seja, um dente do próprio indivíduo. Para isso existem alguns critérios, como o bom estado de saúde geral e local do paciente, o sítio receptor sem danos na estrutura e compatível com o dente doador e deve ter boa parte da raiz desenvolvida. Objetivo: relatar um caso clínico de transplante dentário autógeno como opção de reabilitação oral natural e mais econômica. Relato de caso: Paciente do gênero feminino, 20 anos, compareceu a clínica para exodontia do dente 47, com queixa de incomodo e realização prévia de tratamento endodôntico. Ao exame clínico e de imagem foi possível constatar que o elemento dentário 47 estava impossibilitado de restaurar e que o 48 era compatível com tamanho do 47. Em seguida foi realizado exodontia dos dois elementos e exercido o transplante do elemento 48 no sítio do elemento 47, previamente preparado. Após 12 meses de acompanhamento foi possível observar nova formação de ligamento periodontal. Conclusão: Com isso, foi observado que o transplante dentário se torna uma opção terapêutica viável devido ao seu baixo custo e o dente pertencer ao próprio indivíduo.
