“…Over the years, numerous theories on the pathogenesis of ZD have been proposed. Either manometry or fluoroscopy, or a combination of the two, has generally been used to demonstrate the dynamics of this condition in ZD patients [1,4,6]. Although the results of these earlier studies yielded different hypotheses, the most widely accepted theory is that ZD is a pulsion diverticulum caused by increased bolus pressure.…”

“…Although the results of these earlier studies yielded different hypotheses, the most widely accepted theory is that ZD is a pulsion diverticulum caused by increased bolus pressure. This pressure is the result of a malfunction of the upper esophageal sphincter (UES), such as premature contraction, spasms, or poor relaxation during swallowing [1,4,13].Current surgical management of ZD always includes pharyngoesophageal myotomy accompanied by diverticulectomy or diverticulopexy; this treatment provides very good long-term results after surgery [1,8]. The same principle is also used in the endoscopic treatment of ZD.…”

“…However, the presence of achalasia in 5 of 24 patients with pulsion-type diverticula in our study hints at a possible role of achalasia in the development of the diverticulum, e.g. via an elevation of intraluminal pressure due to functional obstruction (5,11).…”

“…Already in 1947, D'Aubreu [6] suggested that esophageal motor dysfunction could be the pathophysiologic basis of diverticula formation. In 1961, Cross et al [5] gave the manometric proof for this hypothesis. Later reports confirmed these findings: Manometrically detected motility disorders were found in 10% (1/11) to 100% (12/12) of patients [4,7,12].…”
