Background. Low immunity, comorbid clinical conditions, and metabolic disorders may be the underlying factors that determine the severity of infection. Diabetes increases the risk of infection and multiple organ damage. In Nepal, the actual burden of fungal infections has not been estimated or is in a limited progress. This study aimed to investigate the status of fungal infection in diabetic and nondiabetic individuals in Bhaktapur, Nepal. Materials and Methods. A total of 670 samples were collected from 134 participants. From each participant, five samples were collected from different sites like an oral wash, toe swab, midstream urine, hair shaft, and nail scrapings. All samples were cultured on Sabouraud dextrose agar. Gram stain was used to observe yeast cells and lactophenol cotton blue stain was used for hyphae. Chlamydospore production by Candida species was observed in cornmeal agar medium by Dalmau Plate method. Candida species isolated were characterized by germ-tube test and differentiated using CHROM agar Candida medium. Candida species isolates were tested for antibiotic susceptibility. Results. Overall, 19.4% of the samples showed fungal growth. The prevalence of fungal infection was higher in diabetic (34.0%) than nondiabetic individuals (4.7%). Fungal growth was found to be higher in oral wash followed by toe, urine, hair, and nail samples. Predominant fungi were Candida species (57.5%), Aspergillus species (28.4%), and Trichophyton species (10.7%). Oral wash, toe, and urine samples in diabetics had a significantly higher fungal prevalence when compared between both groups, p value < 0.05. In Candida isolates, higher resistance was seen against fluconazole 36.8% and ketoconazole 28.9%, whereas other drugs showed low resistance. Conclusion. Diabetic participants are more susceptible to fungal infection than the nondiabetics. Overall, Candida species and Aspergillus species are highly predominant fungi. Candida species are highly resistant to fluconazole and ketoconazole.

Background: Candida is the leading cause of vaginitis, and 75% of women have at least one episode of infection in their lives, with pregnancy being a predisposing factor. If left untreated, vulvovaginal candidiasis (VVC) can lead to chorioamnionitis with subsequent abortion, prematurity and congenital infection of the neonate. We aimed to determine the prevalence of VVC, identify the recent and most frequently occurring species of Candida in pregnant women, and determine the most effective antifungal drug of choice for treatment. Method: A prospective cross-sectional study in which 176 high vaginal swab samples of consented pregnant women visiting the antenatal clinic from February 2018 to April 2018 were subjected to direct gram smear and culture for Candida isolation. Candida isolates were identified using a germ tube test and HiCrome Candida differential agar. Candida isolates were then subjected to a disk diffusion method using fluconazole (25 μg), nystatin (100 units), and voriconazole (1 μg) on Mueller-Hinton agar supplemented with 2% (w/v) glucose and 0.5 μg/ml methylene blue dye to determine the susceptibility pattern as per the guidelines of the Clinical Laboratory Standard Institute (CLSI). Chi-square analysis was used to ascertain the significant association of participants' sociodemographics and clinical presentations to VVC. A univariate logistic regression model was used to identify potential risk factors of VVC.

Hence, expeditious and dependable species identification is crucial for the effective implantation of treatment strategies and the management of diseases. The purpose of the study was to look at the presence of in various Clinical samples taken from cancer patients who are being treated at a tertiary care hospital in South India. The primary objective of this investigation is to ascertain the susceptibility patterns shown by antifungal drugs. This retrospective investigation was undertaken between January 2022 to April 2023. The isolates were subjected to antifungal susceptibility (AFST) by both Conventional (disc diffusion assay) and Automated methods (VITEK-2 compact - AST YSO1). The susceptibility pattern was recorded against antifungal agents like Fluconazole (FLU), Voriconazole (VOR), Amphotericin B (AMB), and Caspofungin. . was identified by conventional method (Chrome ID) as per standard laboratory protocols and by an automated method using MALDI – TOF. Among 215 samples received from cancer patients suggestive of Candidiasis, albicans (64.6%) was the predominant isolate, and the lowest one was (2.25%). Male patients had a higher prevalence of (55.8%). Similarly, the prevalence was higher in patients over 60, with male patients having an incidence of 38.3% and female patients having 23%. By disk diffusion assay and showed 100% sensitivity to all antifungals used in this study. We conclude that the disc diffusion assay is a more cost-effective, user-friendly, and effective screening test than the VITEK 2, and it has the potential to be used as a workable method for Antifungal susceptibility test. The use of Chrome ID for identification of . is cost-effective and reliable. However, MALDI TOF has the advantage of rapidity, providing correct findings promptly. The timely delivery of diagnostic reports facilitates early diagnosis of patients. Therefore, it is recommended in this research to use MALDI TOF for the identification of . and the disc diffusion technique for antifungal susceptibility testing (AFST).
