Introduction: Planification of living donor nephrectomy requires exact knowledge of the renal vascular architecture because of the high prevalence of anatomical variants. After magnetic resonance angiography (MRA) challenged CT angiography for preoperative assessment over the last years, we revisit in this study the reliability of MRA as the sole preoperative assessment for living donor nephrectomy. Method: We compared the radiological findings of MRA performed as the sole radiological procedure in 44 living kidney donors with perioperative anatomy to verify its sensitivity and to validate its systematic use in the preoperative assessment for living donor nephrectomy. Results: 22 anatomical variants were found in 16 patients (16/44 = 36%). In 4 patients, a polar artery (3 superior, 1 inferior) was not seen by MRA and was detected during surgery (open nephrectomy). Supposing the anatomical variants it described on the opposite side of the nephrectomy are real, sensitivity of this technique in this series is 40/44 (91%) for arterial and 100% for venous imaging. Conclusion: MRA as the sole radiological preoperative assessment performed by a single radiologist with specific expertise and preoperatively reviewed with the harvesting surgeon has been validated as the sole radiological preoperative assessment for living donor nephrectomy at our institution.

Multidetector row computed tomography evaluation of the micropig kidney as a potential renal donor

Multidetector row computed tomography (MDCT) provides anatomical information about the kidney and other internal organs. Presently, the suitability of 64-channel MDCT to assess the kidney of healthy micropigs was evaluated. Morphological evaluations of the kidney and the major renal vessels of six healthy micropigs were carried out using MDCT, recording kidney volume and the diameter and length of renal arteries and veins. The mean diameters and lengths of the renal artery were 0.44 ± 0.05 and 4.51 ± 0.55 cm on the right side and 0.46 ± 0.06 and 3.36 ± 0.27 cm on the left side, respectively. The mean diameters and lengths of the renal vein were 1.44 ± 0.52 and 4.22 ± 1.29 cm on the right side and 1.38 ± 0.17 and 5.15 ± 0.87 cm on the left side, respectively. The mean volume of the right kidney was 79.3 ± 14.5 mL and of the left kidney was 78.0 ± 13.9 mL. The data presented in this study suggest that the MDCT offers a noninvasive, rapid, and accurate method for the evaluation of the renal anatomy in living kidney donors. It also provides sufficient information about extra-renal anatomy important for donor surgery and determination of organ suitability.

BackgroundKidney transplantation from living donors has been increased recently. Preoperative evaluation of living donor is important to select the appropriate kidney for transplantation and to decrease donor surgical complications.ObjectivesThe aim of this study was to compare the accuracy of the use of multidetector computed tomography (MDCT) to evaluate vascular anatomy in living kidney donors with traditional angiography.Patients and MethodsA total number of 60 living kidney donors who underwent open surgical approach for transplantation were selected: Kidney anatomy of donors evaluated by CT angiography (group 1) or traditional angiographic examination (group 2). Renal vessels anatomy was compared with surgical findings in both groups.ResultsThe accuracy for detecting number of main renal arteries were not different in both groups which were 96.7% in CT angiography group and 90% in traditional angiography group (P = 0.15). The accuracy for detection of main renal veins were 100% in group 1 and 96.7% in group 2 (P = 0.31).ConclusionsMDCT has the same accuracy as traditional angiography to detect renal abnormalities in living kidney donors.
