Expression of defensins in the lungs in acute respiratory distress syndrome associated with influenza pneumonia SummaryWe investigated paraffin slices of the lungs from 35 people died during the 2009 epidemic of influenza A / H1N1. The exudative stage of acute res piratory distress syndrome (ARDS) was diagnosed in 10 cases, the proliferative stage in 16 and the fibrotic stage in 9 cases. Expression of α defensins (HNP 1-3) in lung tissue was determined by the immunohistochemistry. The highest HNP 1-3 expression level was found during the exudative stage in neutrophils; it was similar in macrophages and alveolocytes type 2. α defensins were also expressed by the epithelium of terminal bronchioles. During the proliferative stage, the HNP 1-3 expression in neutrophils remained at the high level, it was increased in alveolocytes type 2 and was unchanged in macrophages. During this stage, HNP 1-3 additionally expressed by immature alveolocytes and squamous metaplasia epithelial cells in the terminal bronchioles. During the fibrotic stage, the HNP 1-3 expression in neutrophils was reduced but remained the highest in comparison with that in the other cells; it did not changed in the alveolocytes type 2 and the macrophages. In this stage, HNP 1-3 also expressed in proliferat ing fibroblasts. Key words: α defensins, lung tissue, acute respiratory distress syndrome, influenza A / H1N1. РезюмеБыли исследованы парафиновые срезы легких 35 погибших во время эпидемии гриппа A / H1N1. В 10 случаях была диагностирована экссудативная, в 16 -пролиферативная и в 9 -фибротическая стадия острого респираторного дистресс синдрома. Методом иммуно гистохимии в срезах определялась экспрессия α дефензинов (HNP 1-3) клетками легких. Было обнаружено, что в экссудативную ста дию наибольшая экспрессия HNP 1-3 отмечается в нейтрофилах; в макрофагах и альвеолоцитах 2 го типа она одинаковая, кроме того, α дефензины экспрессирует эпителий терминальных отделов бронхиол. В пролиферативную стадию экспрессия HNP 1-3 в нейтрофи лах остается на прежнем высоком уровне, в альвеолоцитах 2 го типа она увеличивается, в макрофагах -остается неизменной. В этот пе риод HNP 1-3 дополнительно экспрессируют незрелые альвеолоциты, эпителий терминальных отделов бронхиол в состоянии плоско клеточной метаплазии. В фибротическую стадию экспрессия HNP 1-3 нейтрофилами уменьшается, но остается самой высокой по сравнению с остальными клетками, в альвеолоцитах 2 го типа и макрофагах она остается на прежнем уровне. В эту фазу HNP 1-3 до полнительно экспрессируют фибробласты в состоянии пролиферации. Ключевые слова: α дефензины, клетки легких, острый респираторный дистресс синдром, грипп A / H1N1.
