Field performance evaluation of the PanBio rapid SARS-CoV-2 antigen assay in an epidemic driven by 501Y.v2 (lineage B.1.351) in the Eastern Cape, South Africa

Abstract: Background: South Africa was the African country most severely affected by the SARS-CoV-2 pandemic during 2020, experiencing 2 waves of infection. During the first wave, diagnostics were largely based on reverse transcription-linked PCR (RT-PCR). The Abbott PanBio antigen test was deployed during the 2nd wave which was driven by emergence of the 501Y.v2 variant. At the time of evaluation in mid-November 2020, 501Y.v2 was the dominant circulating virus in Nelson Mandela Bay, in the Eastern Cape Province. … Show more

“…Multiple dilutions were detected with m2000, Alinity m, ID NOW, BinaxNOW, and Panbio Ag assays in the expected ranges previously observed with other strains (Tables 1-3) [14, 15]. These results confirm the in silico predictions and are consistent with recent Panbio Ag evaluations of the B.1.1.7 and B.1.351lineages[16,17].…”

“…In 4 cases [49,60,65,66], authors reported that the required number of individuals who tested positive or negative by the reference standard was not reached, and in 1 case [56] no data on age were recorded. Eventually, author queries led to the inclusion of 8 further studies [48,50,51,54,61,63,64,67] resulting in a total of 17 relevant studies for this review (12 peer-reviewed journal articles and 5 preprints). The full list of included studies is reported in Table 3 .…”

“…We identified 10 studies which contributed data relevant to modifying infection-control procedures which are summarized in Table 4. Of these 10 studies, 8 were modeling, 50, 5962 lab-based 63, 64 or grey literature 65 and thus not critically appraised. Of the two that were appraised, one was high quality, 27 and the other was medium quality.…”

“…Between November 17 and 20, 2020, Akingba et al tested the field performance of the PanBio SARS-CoV-2 Rapid Antigen Test (RTD) with 677 patients attending one of 6 mobile clinics in Nelson Mandela Bay, South Africa. 64 This rapid test is available and validated for use in Canada. At this time, South Africa was experiencing their second wave of the pandemic and B.1.351 was responsible for 84% of infections in Nelson Mandela Bay.…”
