First clinical experience of the safety and feasibility of total subcutaneous implantable defibrillator in an Asian population

Abstract: Our initial experience shows that S-ICD is a feasible treatment for VT among an Asian population with smaller body-build. There was nonetheless a relatively high rate of wound complications.

“…Procedure time, a factor that influences infection risk, was similar in a direct comparison of S-ICD with TV-ICD implantation 18. A two-incision technique may lower wound complication rates and has not been associated with increased lead displacement or migration over 12 months of follow-up in over 100 patients 16. Submuscular device placement may reduce risk of erosion, although this has only been demonstrated in a small randomised single-centre study 25.…”

“…Other authors describing patient populations of a similar limited size, and a relatively short follow-up reported a higher percentage of complications, mainly of surgical nature, associated with delayed wound healing or local infection. This type of complications was not observed in patients primarily implanted in our centre, but we reported one such event in a patient with a system implanted in another centre [12,13]. Based on the available data, the initial experience of any centre and the first dozen or so procedures is associated with increased risk of any complications, and only after 13 procedures the performance stabilises [14].…”

“…However, none of these studies involved Asian populations, who generally have lower BMI and a different lifestyle. The mean BMI of an Asian population was reported as 23.0 kg/m 2 [ 15 ], considerably lower than 30.2 kg/m 2 in UNTOUCHED [ 10 ] and 27 kg/m 2 in EFFORTLESS [ 16 ]. S-ICD started clinical use in China in 2015, but only a few retrospective analyses with limited cases were published to demonstrate the procedural characteristics and short-term performance [ 17 – 20 ].…”
