Ganser syndrome and lesion in the temporoparietal region

Abstract: Ganser syndrome is a rare dissociative disorder. It has been reported in association with various functional psychiatric disorders and organic states, most often in patients with head injury and stroke, especially those involving the frontal lobes. The present case of Ganser syndrome had features of hysterical dissociation but was found to have haemorrhage in the temporoparietal region of the dominant hemisphere. The complexities of Ganser syndrome in the presence of an organic lesion with an overwhelming emot… Show more

“…Ouyang et al suggest that emotional stress can produce glutamate in the corticolimbic tract [ 1 ]. Therefore, hyper-glutaminergic transmission in the frontal lobe may cause dissociative symptoms in GS [ 18 ]. Kroll et al postulated that damage to the prefrontal cortex which accounts for confabulation and autobiographical amnesia may result in GS [ 19 ].…”

“…There are reports of Ganser's syndrome secondary to hemorrhage in the temporo-parietal lobe. 7 There are also evidences in literature to support both organic and hysterical reasons for Ganser's syndrome. In our patient the intracranial space occupying lesion in the frontotemporoparietal region was completely resected, which resulted in complete resolution of the symptoms.…”

“…Ganser's syndrome has also been associated with various other functional psychiatric disorders and organic states. 7 This condition is also seen secondary to stroke and head injury, predominantly affecting the frontal lobes. There are reports of Ganser's syndrome secondary to hemorrhage in the temporo-parietal lobe.…”

“…There are few case reports from India showing the finding of approximate answers. [ 2 3 4 5 ] Controversy is centered around the diagnosis and malingering is a very close differential for it. Here, the authors report a case in a nonprison setting which also illustrates the diagnostic difficulty in between the two, especially in view of constraints in terms of changing clinical pictures, noncooperation of patient for detailed psychological assessments and informants.…”
