Abstract:In our experience, retrolaminar optic nerve involvement, with free resection line, and massive choroidal invasion significantly increase the risk for orbital and/or metastatic disease.
“…Massive choroidal invasion is generally considered the only other risk factor, the association of optic nerve and choroidal invasion carrying the greatest risk (Shields et al, 1993a;Khelfaoui et al, 1996;Chantada et al, 1999).…”

“…As we now know which tumors are likely to recur in the orbit on the basis of the histological assessment of the enucleated eye, those at risk can be treated with radiotherapy and/ or chemotherapy adjunctively to minimize the risk [7,11,12,14,19,20]. Brachytherapy is a very good approach to irradiate small volumes precisely [15,22].…”
Insertion of a prosthetic implant at the time of enucleation greatly enhances the subsequent cosmetic appearance. This should be encouraged unless there is frank tumor in the orbit. Orbital brachytherapy without the central train continues to give excellent local control. The short treatment time and good cosmesis are added advantages. The patient is spared the expense and inconvenience of removing and replacing the prosthetic implant.
