Hydrophilic acrylic intraocular lens optic opacification in a diabetic patient

Abstract: We believe this report of calcification of the Akreos(®) MI-60 IOL is of clinicopathological importance. Long-term follow-up of diabetic patients implanted with this IOL should be maintained by surgeons and manufacturers.

“…Reports suggest some systemic diseases as etiologic agents for late IOL opacification, but do not confirm their hypotheses. 9,14,15,16 We found a significant number of patients with arterial hypertension in this study, but there was no correlation between the groups with and without IOL opacification, after 4 years of follow up. Hypertension is a common finding in a senior population; however, detailed investigations of blood and aqueous chemistry in these cases would be helpful in identifying metabolic disturbances that might contribute to dystrophic calcification, mainly if it plays a role in the breakdown of the blood-aqueous barrier.…”

“…5 However, foldable IOLs may present difficulties in IOL centration 6 or develop glistenings and undergo opacification. [7][8][9][10][11][12] In 2012, a hydrophobic foldable IOL with ring-shaped connecting bridges (Figure 1, A) between the haptic and the optic (enVista MX60, Bausch & Lomb) became available. Its design allows the surgeon to use the ringshaped connecting bridges for new fixation suture sites.…”

“…We noticed a higher rate of diabetes in patients with opacification of Akreos Adapt AO IOLs (four out of six patients having concomitant diabetes). Previous studies also supported that IOL opacification was most common in patients with systemic diseases such as diabetes [ 9 , 10 ]. First of all, in cases of diabetic retinopathy (DR), where many pathological conditions such as ischemia/hypoxia, shear stress and inflammation play a role, intravitreal levels of adenosine triphosphate (ATP) are significantly increased as compared with those in non-diabetic controls [ 11 ].…”
