Hypophosphatemic Osteomalacia Due to Drug-induced Fanconi's Syndrome Associated with Adefovir Dipivoxil Treatment for Hepatitis B

Abstract: We herein present the case of a 58-year-old Japanese man with Fanconi's syndrome with a 13-month history of bone pain in his ribs, hips, knees and ankles. He had been receiving low-dose adefovir dipivoxil (ADV) for the treatment of lamivudine-resistant chronic hepatitis B virus infection for eight years and subsequently developed severe hypophosphatemia and proximal renal tubule dysfunction. Magnetic resonance imaging showed multiple insufficiency fractures in the ribs, ileum, tibia and calcaneus. Whole-body b… Show more

“…ADV also shows no cross-resistance with other nucleoside analogs such as lamivudine (LAM), telbivudine, and entecavir (ETV), and thus is widely used as a rescue therapy for these drugs [7,8] . However, renal dysfunction associated with prolonged use of ADV has been reported recently [9][10][11] . For example, in a study of 10 mg ADV combined with 100 mg LAM, serum creatinine increased in 38% of patients following median treatment duration of 38 mo [12] .…”

“…Notably, the patient already had osteomalacia associated with Fanconi's syndrome, although she showed only moderate hypophosphatemia and slightly elevated serum levels of ALP and creatinine, which were not necessarily easy to attend to. In fact, ADV-induced osteomalacia is reported to be typically diagnosed by the incidence of bone fractures (8,9); thus the present case is extremely rare in the sense that osteomalacia was found without any sign of bone fractures.…”

“…She exhibited no sign of bone anterior posterior fractures when she was referred to our hospital, whereas in the majority of previous reports, an episode of bone fracture afforded the first clue for elucidating the adverse effects of ADV therapy (8). Notably, the patient already had osteomalacia associated with Fanconi's syndrome, although she showed only moderate hypophosphatemia and slightly elevated serum levels of ALP and creatinine, which were not necessarily easy to attend to.…”

“…From a point of view of clinical relevance, osteomalacia, which is manifested as bone pain and may contribute to fractures, is regarded as an important side-effect of adefovir therapy. [31][32][33] 29) In conclusion, the present study indicates that 1,25(OH) 2 D 3 treatment can enhance the oral absorption of adefovir dipivoxil (i.e., increased AUC of adefovir, a Mrp4 substrate) via the induction of basolateral Mrp4 function in rat intestine. However, the impact of 1,25(OH) 2 D 3 treatment on the pharmacokinetics of intravenous adefovir is quite limited.…”
