Influence of Narrow Band Imaging (NBI) Videobronchoscopy on the Assessment of Central Lung Cancer Extension and Therapeutic Decision

Abstract: NBI videobronchoscopy had better specificity and sensitivity in the assessment of tumor extension in centrally located lung cancer, and it might play a significant role in the therapeutic decision.

“…NBI, a novel observational method, has facilitated pathological diagnoses of lung cancer by allowing mass observation, guiding targeted biopsies, and enabling predictions of pathological type 30; has improved assessments of extension; and has influenced therapeutic strategies 9,10,14. The results of our meta-analysis suggest that NBI is highly accurate with respect to diagnosing lung neoplasms.…”

“…Four of the included articles 7-10 compared the diagnostic efficacies of NBI and WLB, and three of these articles included data pertaining to patients with of moderate and severe dysplasia. These four studies included a total of 381 patients and 1870 lesions.…”

“…The studies by Shibuya and colleagues [Shibuya et al 2010[Shibuya et al , 2003] were also excluded as no data on sensitivity or specificity were obtainable from these studies. Therefore, for our metaanalyses of diagnostic accuracy, a total of eight studies [Zaric et al 2009[Zaric et al , 2012[Zaric et al , 2013b[Zaric et al , 2013cHerth et al 2009;Bojan et al 2009;Vincent et al 2007;Chen et al 2013] for NBI were included and analyzed. Four of these studies [Chen et al 2013;Herth et al 2009;Zaric et al 2012Zaric et al , 2013c were analyzed to determine the diagnostic performance of combined NBI and AFI.…”

“…A blue narrow band, wavelength ranging from 390 to 445 nm, is absorbed by surface mucosal layer capillaries and a green narrow band, wavelength between 530 and 550 nm, is absorbed by the deeper submucosal thick blood vessels [Herth et al 2010]. Recent studies on NBI have shown higher if not equal sensitivity compared with AFI in the detection of premalignant airway lesions [Bojan et al 2009;Zaric et al 2012]. Specificity has also been shown to be higher in NBI than AFI [Chen et al 2013;Herth et al 2009].…”
