Influence of oestrogen deficiency and excessive mechanical stress on condylar head of mandible

Abstract: Idiopathic condylar resorption (ICR) is a major complication that can occur before or after orthognathic surgery, though the aetiology is unclear. ICR is defined as a change in condyle form, along with decreased condylar head volume and ramus height, and can lead to temporomandibular joint (TMJ) pain and dysfunction (Wolford & Cardenas, 1999). Essentially, excessive mechanical loading including compressive and tractional stress induces change to the form of the condylar head. Ichimiya et al. (2007) investigate… Show more

“…Additionally, the relationship between estrogen and condylar resorption was examined using a rabbit model following an ovariectomy, which indicated that condylar head resorption in the ovariectomy group developed earlier than that in the non-ovariectomy group. Thus, it was concluded that there may be a relationship between estrogen and aggressive condylar resorption (13). Those results identified excessive mechanical stress and estrogen as risk factors for ICR.…”

“…The mandible was exposed with a submandibular approach, then an osteotomy was performed at the midline and the left side of the mandible was moved 3.5 mm forward, then ligated with a 0.4-mm wire and fixed (Figure 1a and b). The amount of forward movement used was based on previous reports presented by the authors (6,13). Fifteen rabbits were randomly divided into three groups and euthanized with an overdose of pentobarbital at one, two, and three weeks after surgery (Figure 1c).…”

“…Oestrogen can regulate the secretion of cytokines and affect some key metabolic pathways to regulate bone and cartilage metabolism ( Wang Q. P. et al, 2012 ). Several animal studies have confirmed that oestrogen deficiency leads to cartilage degeneration in the condyle, and results in more severe TMJOA-like lesions in the presence of mechanical stress stimulation ( Nogami et al, 2020 ; Zhang J. et al, 2021 ). More studies are needed to further explore the role of oestrogen in the pathogenesis of TMJOA and related molecular mechanisms.…”
