Intravenous lobular capillary haemangioma (pyogenic granuloma): A case report and a review of imaging findings as reported in the literature

Abstract: Intravenous lobular capillary haemangioma has distinctive ultrasound but less consistent MRI features although radiological diagnosis should usually be possible. Review of reported cases shows that a previously described gender bias is incorrect.

“…We searched PubMed from January 1979 to August 2020 for "intravascular lobular capillary hemangioma" and "intravascular pyogenic granuloma," which yielded 44 papers, including nine reports on ultrasonography. [9][10][11][12][13][14][15][16][17] Grayscale ultrasonographic features of ILCH were almost uniformly described as a well-defined mass within the lumen of a vein, except for one case where ultrasound failed to identify the host blood vessel. 12 Although some lesions were reported as iso-echoic, most were mainly hypoechoic.…”

“…Similarly, pyogenic granulomas have been described in respiratory, gastrointestinal, ocular, and genitourinary mucosa, sometimes associated with anemia related to chronic bleeding . Other less frequently reported locations are the subcutis and inside large veins . It usually appears in isolation, although cases of multiple pyogenic granulomas have been described, especially with recurrences after treatment, as satellite lesions .…”

“…2 Other less frequently reported locations are the subcutis 3,4 and inside large veins. 5,6 It usually appears in isolation, although cases of multiple pyogenic granulomas have been described, 7-10 especially with recurrences after treatment, as satellite lesions. 11 Pyogenic granulomas have been described in association with mild chronic irritation, previous trauma, insect bites, burns including scalds 10 immunosuppressants, retinoids, and epidermal growth factor receptor or rapidly accelerated fibrosarcoma kinase B inhibitors.…”

“…Intravenous pyogenic granuloma (IVPG) or intravenous lobular capillary haemangioma (ILCH) is a rare, benign tumor usually occurring in the head and neck region 1 . So far, only three cases of patients with IVPG located in the renal vein and thus mimicking renal cell carcinoma morphologically have been described 2, 3, 4, 5. In all patients radical nephrectomy was performed.…”
