Kerion and dermatophytic granuloma. Mycological and histopathological findings in 19 children with inflammatory tinea capitis of the scalp

Abstract: Kerion Celsi is an inflammatory or suppurative type of tinea capitis caused by zoophylic dermatophytes (M. canis and T. mentagrophytes), but also by antrophophylic (T. tonsurans and T. rubrum) and geophylic (M. gypseum) dermatophytes. Histopathological findings showed a spectrum from mild suppurative folliculitis to dense granulomatous infiltrates without a clear relationship with the clinical features.

“…Other fungi associated with MG include Trichophyton mentagrophytes [12], T. violaceum (reported in Europe, affecting scalp) [11••], Trichophyton tonsurans (reported in Latinos, also affecting scalp) [6,13,14], Trichophyton verrucosum [15], Trichophyton schoenleinii, Microsporum canis [6,16], Microsporum audouinii [7], Microsporum gypseum [6,7], Microsporum ferrugineum [7], Epidermophyton floccosum [17], and a yeast-like fungus Trichosporon cutaneum (formerly Trichosporon beigelii) [18,19]; in glabrous skin, the etiology is similar worldwide: T. rubrum (85 %) and T. mentagrophytes var. mentagrophytes (10 %).…”

“…Compared to other dermatophyte species, T. mentagrophytes var. erinacei causes well-circumscribed, severe suppurative inflammation with follicular pustules known as kerion (Arenas et al 2006;Menelaos 2006;Nenoff et al 2014;Sidwell et al 2014). Kerion also resembles a suppurative, moist and painful boggy lesion as the result of advanced disease caused by the dermatophyte organism (Miletta et al 2014).…”

“…Unlike inflammatory tinea or kerion, which result from a hypersensitivity reaction to an infection induced by dermatophytes, MG usually starts as a suppurative folliculitis that may terminate in a granulomatous reaction [3]. …”
