Laparoscopic splenectomy for solitary splenic tumors

Abstract: Splenic solid tumor is a rare entity. Most of the cases were eventually diagnosed as malignant tumors. In our series, all malignant tumors were non-Hodgkin lymphoma. The most common benign lesion was inflammatory pseudotumor. This study has demonstrated the feasibility and safety of LS for diagnosis and treatment of both benign and malignant tumors of the spleen.

“…27,28 Some investigators prefer conservative treatment in the case of small asymptomatic lesions detected incidentally, reserving splenectomy for large, multiple, or symptomatic lesions. 27,29 Partial resection techniques have also been applied in cases of limited disease; however, leaving a splenic remnant in cases of diffuse involvement increases the risk of further growth and enlargement, sometimes requiring a second operation.…”

“…Bleeding from splenic hilum and bleeding due to enlarged lymph nodes in the splenic hilar region were reported as causes for conversion to open in few studies [8,9] . None of the patients in this series had relaparotomies as a result of postoperative bleeding as it was reported in few studies [2] . The average length of stay was comparable with other studies, excluding the one patient with combined umbilical hernia repair .…”

“…Since then it has been recognized as an accepted procedure for elective splenectomies in the management of various hematological as well as neoplastic conditions [1,2]. Though, many major reports were available in the international arena, not much was reported from the southern part of India.…”

“…According to the literature, the right lateral decubitus approach is preferred since this position increases exposure, improves dissection, and possibly reduces the number of trocars required for the operation [14]. The use of a laparoscopic vascular stapler, electrothermal bipolar vessel sealer, and ultrasonic knife are options to facilitate the surgical steps of hemostasis, but these devices increase the cost [15]. The recurrence rate is low, i.e., 9.5 % of patients, frequently after incomplete resection.…”
