Levonorgestrel Implants Enhanced the Suppression of Spermatogenesis by Testosterone Implants: Comparison between Chinese and Non-Chinese Men

Abstract: T implants resulted in more pronounced spermatogenesis suppression in Chinese men. Addition of LNG implants to T implants enhanced the suppression of spermatogenesis in the treatment period in both Chinese and non-Chinese men.

“…Monthly injection of testosterone undecanoate in Chinese men did not cause headaches [9,35,45]. Headache was reported in 5% of men in a short term study using testosterone and Nestorone gel treatment for 20 days [33] however other studies which used combination of testosterone and progestin combinations did not report headache as a side effect [31,44,46,48,59]. Because non-specific headaches are common occurrence in healthy men, the issue whether hormonal contraception has any causal effect on headaches should be demonstrated in placebo controlled studies.…”

“…With the suppression of spermatogenesis, seminiferous tubule volume decreases resulting in reversible decreases in testicular volume [36,37]. Testis volume decreases usually by 4-5 ml [9,34,38]. This decrease in size of the testis is usually not reported by most men participating in contraceptive studies.…”

“…[53][54][55] In Chinese men, injectable testosterone undecanoate (TU) alone or in combination with oral levonorgestrel (LNG) resulted in the relatively slow suppression of spermatogenesis. 56 Thus, this study provides an ideal model with hormone-induced suppression of spermatogenesis.…”

“…In addition, there are ethnic differences in the response of sperm concentrations to male contraceptive regimens. For example, study volunteers in Asia exhibit rates of azoospermia in the 90 -100% range on testosterone-alone regimens, whereas Caucasian men have rates of azoospermia closer to 60% with the same treatment (142, [213][214]216). The explanation for this difference is unknown, although it is unlikely to be due to different rates of metabolic clearance of testosterone (122), and it complicates extrapolation of rates of suppression of sperm concentrations between populations.…”
