Malignant Lymphoma Demonstrating Sick Sinus Syndrome.

Abstract: ereport a case of T-cell-rich large B-cell lymphomademonstrating sick sinus syndrome as a single initial symptom, followed by the retention ofpericardial and pleural effusions. Intrapleural administration of interferon-a prevented reaccumulation of the pleural effusion for one and a half months, whereas systemic chemotherapy failed to control tumor growth in the skin and lymph nodes. The autopsy revealed involvement of the sinoatrial node of the heart by lymphomacells.

“…Sick sinus syndrome is an uncommon complication of cardiac lymphoma and could be an initial symptom or a terminal manifestation ( 5 ). Clinical presentations include sinus arrest, tachy-brady syndrome, or AF with slow ventricular rate ( 6 ). Most cases have been of B-cell origin, whereas only 1 case was of T-cell origin ( 7 ), and pathology examination demonstrated typical infiltration of lymphoma cells in the sinus node region ( 5 ).…”

“…Common symptoms include heart failure, pericardial effusion, atrioventricular block, SVC syndrome, and precordial pain, but there have been few cases of sick sinus syndrome in previous reports. [6][7][8] Our patient presented with sick sinus syndrome and developed SVC syndrome, caused by the rapidly grown B-cell malignant lymphoma.…”

“…The presentation of sick sinus syndrome is a rare occurrence in comparison to atrioventricular conduction disturbances (5). Although sick sinus syndrome is mostly caused by degenerative fibrosis of the sinus node associated with aging, infiltration of leukemia or cancer, amyloidosis, fatty replacement, arteritis, and myocarditis have been re- ported as the cause of sick sinus syndrome as well (6). Therefore, the possibility of cardiac involvement of lymphoma should be acknowledged in patients presenting with sick sinus syndrome with unknown causes (6).…”
