Background and aimsMalnutrition is a prevalent problem occurring in different diseases. Hemorrhagic transformation (HT) is a severe complication of acute ischemic stroke (AIS). Few studies have evaluated the association between malnutrition risk and hemorrhagic transformation in patients with acute stroke. We aim to investigate the influence of malnutrition risk on the risk of hemorrhagic transformation in patients with AIS.MethodsA total of 182 consecutive adults with HT and 182 age- and sex-matched patients with stroke were enrolled in this study. The controlling nutritional status (CONUT) score was calculated to evaluate the malnutrition risk. HT was detected by follow-up imaging assessment and was radiologically classified as hemorrhagic infarction type 1 or 2 or parenchymal hematoma type 1 or 2. Logistic regression models were conducted when participants were divided into different malnutrition risk groups according to the objective nutritional score to assess the risk for HT.ResultsThe prevalence of moderate to severe malnutrition risk in patients with AIS was 12.5%, according to the CONUT score. Univariate analysis showed that the CONUT score is significantly higher in patients with HT than those without HT. After adjusting for potential covariables, the patients with mild risk and moderate to severe malnutrition risk were associated with a higher risk of HT compared to the patients in the normal nutritional status group [odds ratio, 3.180 (95% CI, 1.139–8.874), P = 0.027; odds ratio, 3.960 (95% CI, 1.015–15.453), P = 0.048, respectively].ConclusionMalnutrition risk, measured by CONUT score, was significantly associated with an increased risk of hemorrhagic transformation in patients with AIS.

Our objective was to identify the optimal method to assess reduced muscle mass (RMM) using the Global Leadership Initiative on Malnutrition (GLIM) approach and investigate the roles of the GLIM approach in nutrition assessment and survival prediction in colorectal cancer (CRC) patients. During a median follow-up period of 4.2 (4.0, 4.4) years, a development cohort of 3612 CRC patients with a mean age of 64.09 ± 12.45 years was observed, as well as an external validation cohort of 875 CRC patients. Kaplan–Meier curves and multivariate Cox regression were adopted to analyze the association between GLIM-diagnosed malnutrition and the overall survival (OS) of CRC patients. A nomogram predicting individualized survival was constructed based on independent prognostic predictors. The concordance index, calibration curve, and decision curve were applied to appraise the discrimination, accuracy, and clinical efficacy of the nomogram, respectively. Patients diagnosed with severe malnutrition based on either the mid-arm muscle circumference (MAMC) or body weight-standardized hand grip strength (HGS/W) method had the highest mortality hazard ratio (HR, 1.51; 95% CI, 1.34–1.70; p < 0.001). GLIM-defined malnutrition was diagnosed in 47.6% of patients. Severe malnutrition was an independent mortality risk factor for OS (HR, 1.25; 95% CI, 1.10–1.42; p < 0.001). The GLIM nomogram showed good performance in predicting the survival of CRC patients and was clinically beneficial. Our findings support the effectiveness of GLIM in diagnosing malnutrition and predicting OS in CRC patients.

OBJETIVO: Descrever o perfil sociodemográfico das mortes por desnutrição no Brasil entre os anos de 2010 e 2020. MATERIAIS E MÉTODOS: Trata-se de um estudo ecológico de corte transversal, de caráter descritivo, com abordagem quantitativa realizado por meio do Departamento de Informática do Sistema Único de Saúde (DATASUS), com dados de 2010 a 2020 oriundos do Sistema de Informação sobre Mortalidade (SIM), sendo selecionadas as variáveis sociodemográficas, causa do óbito e desfecho, esta última referente ao local de ocorrência. RESULTADOS E DISCUSSÃO: Foram registrados 45.632 óbitos por desnutrição, sendo o número médio de mortes por ano de 4.148 óbitos. Dos óbitos computados, 51% são do sexo masculino, 47,44% de cor/raça branca, seguido pela parda com 41,32%, sendo que do número total de casos 86,33% ocorreram com idosos. Ao analisar o nível de instrução, observa-se uma relação inversamente proporcional entre o número de óbitos por desnutrição e escolaridade dos indivíduos, dado que quanto maior o tempo de estudo menor o número de óbitos. CONCLUSÃO: Observa-se que a maioria das mortes por desnutrição aconteceu entre homens e idosos, com indivíduos divorciados e dentre aqueles com baixo nível de escolaridade.
